- Witness statement for court, model to copy by hand (France)For someone giving written evidence in a civil case in France: a dispute between neighbours, at work or in the family, an accident. Article 202 of the French Code of Civil Procedure requires the statement to be written, dated and signed in the witness's own hand, so this document is not the statement itself but a model in French, filled with your details, to copy by hand on plain paper or on the official form Cerfa 11527, which is free and optional. It holds every compulsory statement. Enclose a copy of a signed ID document.
- Referral to the CADA after a refused document request (France)For someone to whom a French administration refused a document, or did not answer within a month. The Commission d'accès aux documents administratifs (CADA) is approached free, preferably through the online form on its website. This letter, in French under French law, refers the matter by post: it names the administration, the date of the request, the documents and the answer received, and asks for a favourable opinion. Referral within two months of the refusal is compulsory before the administrative court.
- Sworn statement — attestation sur l’honneur (France)For declaring in writing, on your own responsibility, a fact about yourself when an administration, employer, school, bank or landlord in France asks for it: family situation, lack of income, residence, a lost document, the accuracy of information. In French under French law, the statement identifies the declarant, sets out the facts, names the recipient if you wish and recalls the penalties for a false statement (article 441-7 of the French Criminal Code). For evidence in court, use the witness statement instead.
- Power of attorney for a specific act (France)For asking someone to do one specific thing in your place when you cannot be there: collect a document, sign a lease or a contract, sell or buy a vehicle, represent you at an appointment. You describe the act, and the power covers that act only, until the date you choose (articles 1987 and 1989 of the French Civil Code). A voting proxy is made free on maprocuration.gouv.fr, and, for an act before a notary, the notary provides the power of attorney required. Signed by the principal, with a copy of their ID.
- Revocation of a power of attorney, with notice to third parties (France)For ending a power of attorney, general or for one act, given to a relative or anyone else. The French document holds two letters, sent the same day by registered post with proof of receipt: the revocation to the agent, asking for the original back and an account of what was done (articles 1993 and 2004 of the French Civil Code), and the notice to the bank or body that knows of the power. Without that second letter, third parties unaware of the revocation may still rely on the power (article 2005).
- Designation of a trusted person — personne de confiance (France)For any adult who wants to name a relative, someone close or their GP as their trusted person: that person accompanies them in their dealings and medical appointments if they wish, and is consulted if they can no longer express their will (article L. 1111-6 of the French Public Health Code). The designation is written and co-signed by the person named; hospitals offer their own free form on admission. This French document is sold blank, filled in by hand, and revocable at any time.
- Sworn statement of accommodation — attestation d’hébergement (France)For someone in France who houses a relative, friend or flatmate with no bills in their own name: the attestation d’hébergement serves as the housed person's proof of address with the préfecture, the town hall, the CAF or a bank. In French under French law, it identifies the host and the person housed, the arrival date and whether the host owns or rents, and recalls the penalties for a false statement (article 441-7 of the French Criminal Code). Signed by the host, with a copy of the host's ID and proof of address.
- PACS agreement — civil solidarity pact (France)For two adults about to enter a PACS at the town hall (before a notary, the notary drafts the agreement). A PACS is declared on Cerfa form 15725, and a free model agreement exists (Cerfa 15726), on service-public.gouv.fr: you do not need DocMuse to enter a PACS. Our own French agreement refers to the law and to articles 515-1 to 515-7 of the French Civil Code and lets you choose the material support and the property regime: separate property or joint ownership. Signed by hand on paper: article 1175 of the French Civil Code excludes the electronic form.
- Covering letter for a change of first name (France)For a person asking to change their first name, or that of a minor over whom they exercise parental authority. The application itself is made on Cerfa form 16233 (adult) or 16234 (minor), free on service-public.gouv.fr: you do not need DocMuse for it. This French letter goes with the form: it sets out the legitimate interest the civil registrar weighs (article 60 of the French Civil Code) and lists the proof enclosed. It is filed at the town hall of residence or of the place of birth, often by appointment.
- Request for administrative documents (France)For obtaining from a French administration — town hall, préfecture, public hospital, school, public body — a copy of a document it holds: a decision, a report, a file, minutes. The letter, in French under French law, cites the right of access (articles L. 311-1 ff. of the French Code of Relations between the Public and the Administration), lists the documents, the chosen form of access and partial disclosure, and recalls that one month's silence counts as refusal, opening referral to the CADA. Send it by registered letter with proof of receipt.
- General power of attorney for one's affairs (France)For handing the day-to-day running of your affairs to someone close during an absence, a hospital stay or a stay abroad: paying bills, collecting income, dealing with public offices. Written in general terms, it covers acts of administration only (article 1988 of the French Civil Code); selling or giving away property needs an express power. To arrange your own protection in advance should your faculties decline, the mandat de protection future (Cerfa 13592 or a notarial deed) is the route the law provides. Signed by principal and agent, in two copies.
- Cohabitation agreement for an unmarried couple (France)For an unmarried couple without a PACS who live together in France and want to set their own rules in writing, since the law provides almost none (article 515-8 of the French Civil Code): who owns the furniture and purchases, how rent and expenses are shared, what happens to the home and joint property on separation. A list of each partner's property can be attached. Signed by both partners, in two copies. It creates no right to inherit from the other.
- Advance directives on end-of-life care (France)For any adult who wants to make known their wishes on continuing, limiting or stopping treatment should they one day be unable to speak for themselves. The French health ministry publishes a free official model, set by the arrêté of 3 August 2016: you do not need DocMuse to write directives. This French document is sold blank and filled in by hand: wishes, trusted person, witnesses where needed. Directives bind the doctor, save the exceptions of article L. 1111-11 of the French Public Health Code, and can be revoked at any time.
- Other parent's consent to a minor's usage name (France)For the parent who agrees to the usage name chosen for a minor, to be printed on the child's identity card or passport. A free model of the parents' agreement, and a model of the child's consent, are annexed to the French justice ministry's circular of 15 June 2023, also on justice.fr: you do not need DocMuse to get them. The parent who did not pass on their name may add it alone, after telling the other (article 311-24-2 of the French Civil Code); both parents' agreement is needed for other choices, such as using one name instead of the other. Signed by the other parent, and by a child over thirteen.
- Sworn declaration of cohabitation (France)For an unmarried couple without a PACS who are asked to prove they live together: by an insurer, a health fund, an employer, a landlord, a pension fund or a foreign authority. Both partners declare on their honour that they live together in a stable and continuous way (article 515-8 of the French Civil Code), since when and at which address, and enclose their ID and proof of address. The French document recalls the penalty for a false declaration (article 441-7 of the French Criminal Code).
- Parenting agreement: residence, contact and child maintenance (France)A standard form for separated parents, married or not, who agree on how their children's lives are organised: alternating residence or residence with one parent, contact and staying rights, holidays, maintenance and education costs, exceptional expenses. To have their agreement approved, parents apply to the family judge on Cerfa form 16139, free on service-public.gouv.fr, which also offers two model agreements: you do not need DocMuse for this step. Our form lets the parents choose each clause and does not review their situation. Unmarried parents can also have their maintenance agreement made enforceable by the CAF or MSA.
- Parental consent to medical care and treatment of a minor (France)For parents who leave their child with grandparents, a childminder, a holiday centre or a school trip. The French document authorises the named person or body, for a set period, to have the child treated and, in an emergency, to allow whatever operation the doctor considers necessary if the parents cannot be reached in time (article R. 1112-35 of the French Public Health Code). It holds no medical data. Signed by one parent, or by both for a planned operation.
- Invitation letter for a visa application (France)For someone in France inviting a foreign relative or friend who needs a visa, for a private visit of under three months. The official document is the attestation d’accueil (Cerfa 10798), which the host requests in person at the town hall, with a €30 tax stamp. This letter, in French under French law, goes with it in the visa file without replacing it: it presents the guest, the dates and purpose of the stay, the accommodation and the costs the host covers. The guest attaches it to the visa application.
- Administrative appeal against a decision — recours gracieux or hiérarchique (France)For challenging a decision of a French administration — a refused application, a penalty, a decision of a town hall or préfecture — by asking its author (recours gracieux) or the author's superior (recours hiérarchique) to withdraw or change it. Lodged within two months, the appeal interrupts the deadline for the administrative court (article L. 411-2 of the French Code of Relations between the Public and the Administration), except in planning matters; two months' silence counts as rejection. The letter, in French, sets out the decision, your grounds and your request. Send it by registered post.
- Parental authorisation for a minor travelling with another adult (France)For entrusting a child to a grandparent, a relative or a holiday organiser for a trip. If the child leaves France with no holder of parental authority, the only parental authorisation required is the exit authorisation (AST, Cerfa form 15646*01), free on service-public.gouv.fr: you do not need DocMuse for it. This French letter goes with it, or serves for a trip within France: it names the companion, the dates and the destination, and gives a contact. A child travelling with one of its parents needs no authorisation at all.
