The application to a German long-term care insurer (Pflegekasse) for care-insurance benefits and the assessment that sets a care level (Pflegegrad) — for yourself or, with a power of attorney or a carer's card from the court, for someone else. An informal application is enough, but only a written one proves the date from which benefits run (§ 33 of Book XI of the Social Code, SGB XI). It flags the shorter deadlines after a hospital stay or announced care leave, and adds a ready letter claiming €70 per week begun if the insurer misses its 25 working days. In German.

Common questions
- From when does the German care insurer pay once a care level is granted?
- From the month in which the application reaches the insurer, though not before the conditions are met (§ 33(1) SGB XI). So it pays to apply at once, even if not every document is ready. A call or an e-mail is enough in law, but only a written application with a confirmation of receipt proves the date reliably.
- What happens if the German care insurer takes too long?
- The insurer must decide in writing within 25 working days of receiving the application. If the person is in hospital or rehabilitation, or a family carer has announced care leave, shorter assessment deadlines apply. If the insurer misses the deadline and is responsible for the delay, it pays €70 for each week begun (§ 18c SGB XI). The second letter prepared in this document claims the payment; fill in the place and date by hand.
- Can the German care-level application be made for a parent or another person?
- Yes, with a power of attorney covering care-insurance matters, or if the guardianship court has appointed you for that area. Choose the matching statement in the document, enclose a copy of the power of attorney or the carer's card, and sign in your own name. People with private long-term care insurance send the application to their insurance company; the €70 rule applies to private compulsory care insurance in the same way (§ 18c SGB XI), so the second letter works there too.

The application asking a German statutory health insurer (Krankenkasse) to pay for treatment, a medical aid or another service — before treatment, as a request for cost cover with the three-, five- or six-week decision deadline of § 13(3a) of Book V of the Social Code (SGB V), or afterwards, as a claim for reimbursement where the insurer missed the deadline, could not provide urgent care in time or wrongly refused it. - Objection to a decision of the health or care insurer (Germany)
The objection (Widerspruch) to a decision of your German statutory health or long-term care insurer (Krankenkasse or Pflegekasse) — for instance refusing treatment, a medical aid or rehabilitation, or setting too low a care level. With the decision's date, the reference, your reasons and a request to see the file, so you can add to the reasons once you have read the expert opinion. To print, sign and post: under § 84 of the Social Courts Act (SGG) a plain e-mail is not a valid objection. Deadline: one month. In German.
