Parental consent to a passport for a child (Poland)

A parent's written consent to a Polish passport being issued to a minor, with the details art. 14 of the Passport Documents Act requires: the parent's name, PESEL number and identity document, and the child's name and PESEL. On paper it counts only once the signature is certified by a notary or a passport authority — a voivode's office or a consul; the document leaves room for that.

How can a parent consent to a child's passport without going to the office together?

In three ways: in person at any passport authority — a voivode's office in Poland or a consul abroad; in writing, with the signature certified by a notary or a passport authority; or electronically, signed with a qualified, trusted or personal signature, for example through the e-service on gov.pl (art. 14 of the Passport Documents Act). The consent must contain the parent's and child's details the Act lists.

What if the other parent will not agree to the child's passport?

A family court ruling can replace the other parent's consent; the application goes to the district court for the child's place of residence, and the ruling is attached to the passport application. No consent is needed from a parent the court has deprived of parental authority. A limitation of parental authority does not always remove the need for consent — the wording of the ruling decides.

Is consent to a passport also consent for the child to travel abroad?

No. It covers only issuing the passport. A trip — especially a long one or a move abroad — is decided separately: it is an important matter for the child that the parents settle together, or the guardianship court if they cannot agree (art. 97 § 2 of the Family and Guardianship Code). For a specific trip, a separate consent with the dates and the companion's details is worth having.
