Consent to being photographed and to the use of the image
Needed by photographers, agencies, schools and event organisers — the image of an identifiable person is personal data, and using it for advertising or publication requires consent. The declaration sets out where and for how long the material will be used, whether there is a fee, and whether the person will be named. For a child it is signed by a parent.
Consent to the processing of personal data
Consent is only one of the grounds for processing, and the most fragile one: it has to be specific, informed and capable of being withdrawn, and silence or a pre-ticked box is not consent. So this declaration lists the purposes separately and states the right to withdraw expressly. If you process data under a contract or under a statutory duty, you do not need consent and should not ask for it.
Owner's consent for address registration
When you register at an address that is not yours, the municipality asks for the owner's written consent. This is that document. It goes in together with the application for a permanent or current address, and without it the counter returns the whole file. If the owner will not be coming to the counter in person with their ID card, the signature has to be notarised — which is why most people end up going twice.
Consent to act as managing director and specimen signature
The document by which a managing director accepts the post and gives the specimen signature the Commercial Register will hold. Its whole point is the notarisation — the signature is given in front of a notary, not at home. The lower part of the sheet is left blank for exactly that; do not fill it in.
Consent for a child to travel abroad
When a child leaves the country with only one parent or with another adult, border control asks for the other parent's written consent. The declaration names the countries, the period and the accompanying adult by name. The signature has to be notarised — without that the document is of no use at the border.
