gov.gr issues solemn declarations free; this document gives you the wording of the consent, in full in Greek and in English, to paste there, or to sign as a PDF and have the signature certified free on docs.gov.gr or at a citizen service centre (ΚΕΠ). It is DocMuse's own text, not the Greek state's form. With it the parent who is not travelling consents to their minor child travelling abroad with the other parent or another adult: the parent's, child's and companion's details, destination and dates. Consular guidance asks for it as a solemn declaration (υπεύθυνη δήλωση) with the signature authenticated and, abroad, in English too.

How is the bilingual Greek travel consent for a minor used with gov.gr?
gov.gr issues a solemn declaration free of charge, with the force of one bearing an authenticated signature (Greek Law 4727/2020): paste the Greek and the English text into its free-text box. To keep the document as it is, upload the PDF to docs.gov.gr, which certifies the signature digitally, or sign the printed copy at a citizen service centre (ΚΕΠ) — not at the police, since 2022. A gov.gr document carries a verification code and a QR code on every page, with which passport control or an airline can check it on gov.gr. Carry a printed copy as well.
Does a child leaving Greece with one parent need the other parent's consent?
Parental care is as a rule exercised jointly by both parents (art. 1510 of the Greek Civil Code). When a minor travels abroad with one parent or with someone else, consular guidance advises carrying the consent of the parent who is not travelling, as a solemn declaration with the signature authenticated or issued on gov.gr — not a notarial deed. At the Schengen external borders, guards pay particular attention to minors, and airlines may ask for it too. If the child travels with neither parent, each parent signs a declaration; for a child flying alone, the airline has its own form.
Why is the Greek travel consent for a minor also written in English?
Because border authorities and carriers outside Greece read it, and consular guidance asks for it in English too when the child goes abroad. The document has the full text in Greek and in English, with the same details and one signature covering both. Write the names as they appear in the passport. For a country outside the European Union, ask its embassy whether it also wants an official translation or an apostille.
