Parent's authorisation for a minor's medical care by a carer (Greece)
Published byDocMuse
This document is in Greek
The PDF you download is in Greek — that is the language it has to be filed in, so it is not translated. Everything on this page is here to tell you what it says.
gov.gr issues solemn declarations and authorisations free; this document gives you the wording, to paste there, or to sign as a PDF and have the signature certified free on docs.gov.gr or at a citizen service centre (ΚΕΠ). It is DocMuse's own text, not the Greek state's form. With it a parent allows whoever looks after the child in their absence — a grandparent, a school-trip or camp leader — to take the child to the doctor, receive information and agree to routine medical care for a set period, with the child's health details. Under the Greek Code of Medical Ethics, consent for a minor is given by those with parental care.

Common questions
- How is the authorisation for a child's medical care given through gov.gr or a Greek citizen service centre?
- Free of charge, in two ways. On gov.gr, issue a solemn declaration or an authorisation and paste the document's text into the free-text box. Or upload the PDF to docs.gov.gr (digital document certification), which certifies the signature with the force of an authenticated one, or sign the printed copy at a citizen service centre (ΚΕΠ) — not at the police, since 2022. Public bodies must accept gov.gr documents.
- Who consents to medical treatment for a minor in Greece?
- Under art. 12 of the Greek Code of Medical Ethics (Law 3418/2005), consent for a minor is given by those who exercise parental care, and the doctor also takes the child's own view into account according to their maturity. A grandparent or a trip leader has no authority of their own to consent. The parent's authorisation gives it to them for routine care, while for a serious procedure the doctor contacts the parent.
- What happens in a medical emergency in Greece if the child's parent does not answer the phone?
- In an emergency, where the parent's consent cannot be obtained in time, the doctor gives the care needed under the Greek Code of Medical Ethics. The authorisation does not change that rule, but it lets an ordinary illness be dealt with quickly, gives the doctor the child's allergies and medicines, and provides that the parent is told at once.
