The PDF you download is in Greek — that is the language it has to be filed in, so it is not translated. Everything on this page is here to tell you what it says.
The letter in which an insured person in Greece notifies the insurer of a hospital stay, an operation or another insured event and claims the benefit the private health policy provides: the policy, the event, the healthcare provider and dates, the amount in figures and words, the IBAN and the attachments. Law 2496/1997 (art. 7) requires notice within eight days of learning of the event, and this letter gives notice and claims at once. Reimbursement from EOPYY, the public health fund, is claimed on its own forms.

Common questions
- Within how many days must a Greek health insurer be told of a hospital stay?
- Within eight days of the insured learning of the insured event (art. 7 of Law 2496/1997), unless the policy allows longer. Many health policies also ask for notice or approval before a planned admission; the policy terms say so. Late notice can complicate payment, so send the letter at once and let the receipts follow.
- What papers usually go with a claim under private health insurance in Greece?
- The medical ones — the admission and discharge note, the doctor's report, test results — and the financial ones: receipts or invoices in the insured person's name and, if EOPYY or another fund covered part of the cost, the document showing it. The exact list is in the policy terms, and the letter lists what is attached.
- What can the insured do if a Greek insurer refuses to pay a health claim?
- Ask for a written, reasoned answer and complain to the insurer's complaints service. If that fails, Greece's Independent Authority for Market Control and Consumer Protection, which absorbed the Consumer Ombudsman (Law 5255/2025), mediates individual consumer disputes. The last step is court, where this notice and claim prove when the insurer was told.
