This document is in Italian

The PDF you download is in Italian — that is the language it has to be filed in, so it is not translated. Everything on this page is here to tell you what it says.

An Italian letter, in Italian, from a member of a supplementary health fund, a mutual fund or a health insurance policy, claiming back visits, tests, treatment or hospital stays already paid: it lists the invoices and prescriptions enclosed, gives the IBAN and asks to be told in writing of any missing documents. It is for funds that accept claims by post or PEC, or to chase a claim that has stalled; if the fund has its own members’ area or form, use those. It holds health data, so it is sold blank and filled in by hand.

Common questions

By when must a claim be made to an Italian supplementary health fund?
Within the time set by the fund’s rules or the policy, which differ from one fund to another: some want the claim by a given month of the year after the invoice, and incomplete or late paperwork is not refunded. Read your plan’s rules and, if the fund has a members’ area or a free form, use them: this letter is for funds that accept a written claim, or for chasing one.
What can you do if an Italian health insurer refuses the refund?
Ask in writing for the reasons and the clause applied. With an insurance company, make a written complaint to it: if after 45 days there is no answer or it does not satisfy you, you can go to the Arbitro Assicurativo, the Italian insurance ombudsman, within 12 months of the complaint, for a fee of 20 €. A supplementary health fund that is not an insurer follows the complaints procedure in its own rules.
Does the Italian national health service refund private visits?
No: the national health service provides care directly, in its own or contracted facilities, with any prescription charge (ticket). Refunds of private expenses come from supplementary health funds, often set up under collective agreements, from mutual funds and from health insurance policies. Care abroad follows a procedure of the local health authority that this letter does not cover.

Many authorities have their own free complaint form. Send it by PEC, registered post with proof of receipt, or by hand. It holds health data, so it is sold blank and filled in by hand.

The facility must hand them over within seven days (art. 4 of Italian law 24/2017). Many facilities have their own form or a free online service. Send it by PEC, registered post with proof of receipt, or by hand. It holds health data, so it is sold blank and filled in by hand.
