This document is in Italian
The PDF you download is in Italian — that is the language it has to be filed in, so it is not translated. Everything on this page is here to tell you what it says.
An Italian letter, in Italian, to the medical director’s office of a hospital or clinic asking for a copy of the medical record (cartella clinica) and other health documents: the patient, a delegate, a parent or guardian, or an heir gives the stay or treatment, the documents wanted and the format. The facility must hand them over within seven days (art. 4 of Italian law 24/2017). Many facilities have their own form or a free online service. Send it by PEC, registered post with proof of receipt, or by hand. It holds health data, so it is sold blank and filled in by hand.

Common questions
- How long does an Italian hospital have to provide a copy of the medical record?
- Seven days from the request for the documents it holds, and at most thirty days for any additions (art. 4(2) of Italian law 24/2017). Many facilities publish their own form or a free online service, which you can use instead of this letter. In the province of Bolzano a public facility must also accept the request in German, and in the Aosta Valley in French.
- How much does a copy of an Italian medical record cost?
- Each facility applies its own price list to cover copying and postage, and some give the first copy free. The price list also says whether stamp duty is due on copies certified as true copies: many facilities treat them as exempt. Article 15 of Regulation (EU) 2016/679 also gives you the right to a first copy of your own personal data free of charge.
- Who besides the patient can ask for an Italian medical record?
- A person the patient has authorised in writing, with a copy of both identity documents; a parent for a child under 18 and a guardian for a person under guardianship; and the heirs of a deceased patient, who prove their status, for example with a self-declaration under Italian law (dichiarazione sostitutiva). The letter has a line saying in what capacity the request is made.

It is for funds that accept claims by post or PEC, or to chase a claim that has stalled; if the fund has its own members’ area or form, use those. It holds health data, so it is sold blank and filled in by hand.
