The written consent by which a parent lets whoever looks after their child in their absence — a grandparent, a school-trip or camp leader — take the child to a doctor in Cyprus or abroad, receive information and agree to routine medical care for a set period. It gives the doctor the child's allergies, medicines and GESY personal doctor, leaves serious procedures to the parent, and follows the Cypriot Patients' Rights Law, under which a parent consents for a minor and the child's own view counts for more as the child grows older.

- Who consents to medical care for a minor in Cyprus?
- Under the Cypriot Patients' Rights Law (Law 1(I)/2005), where a minor cannot consent, care is given with the authorisation of the parent or another person the law names, and the child's own view is taken into account more and more with age and maturity. A grandparent or a trip leader has no authority of their own. This consent gives it to them for routine care, while for a serious procedure the doctor contacts the parent.
- What happens in a medical emergency in Cyprus if the child's parent cannot be reached?
- Where care is urgent and the parent's consent cannot be obtained in time, the Patients' Rights Law lets the doctor give it, unless it is obvious the parent would have refused. This consent does not change that rule, but it lets an ordinary illness be dealt with quickly, gives the doctor the child's allergies and medicines, and provides that the parent is told at once. For a trip within the European Union, also take the child's European Health Insurance Card, issued by the Health Insurance Organisation.
- Does a consent to a child's medical care also let the child travel abroad from Cyprus?
- No. This consent covers medical care only. When a minor travels abroad from Cyprus with one parent, the other parent's written consent, with a certified signature, is needed even if the parents live together; when the child travels with someone else, such as a grandparent or a trip leader, both parents' consent is needed; and if a parent will not consent, a court order is needed. For such a trip, take both documents along: the travel consent and this one for medical care.

It names the department, the period of treatment, the documents wanted and how to receive them. The first copy is free, and the answer is due within one month, extendable by two months only for complex or numerous requests.
