Application for childcare leave
Leave under Art. 164 of the Labour Code is taken after maternity leave and is granted on an application to the employer, while the benefit is paid by the social security institute through a separate procedure. The age limit and the amount of the benefit are set by law and do change — check them for the year you are applying in. The application also states whether the leave is being transferred to the other parent or to a grandparent, which is common.
Application to terminate employment within the probationary period
While an agreed probationary period is running, the party in whose favour it was agreed may end the contract with no notice and without giving a reason. The application identifies the contract, the termination date, and that the ground is Art. 71 of the Labour Code. Check your own contract for whose benefit the probationary period was agreed before filing it.
Application to change working hours
For moving to part-time hours, for a different start and finish, or for a flexible arrangement. The change affects an essential term of the employment contract, so it is put into effect by a written amendment — and part-time hours reduce pay proportionately. The application proposes a specific arrangement and the date it should start.
Application to withdraw a notice of resignation
A notice cannot be withdrawn unilaterally — once it has been given, withdrawing it needs the employer's agreement. That is why this document is an application rather than a notification, and why it should be filed as early in the notice period as possible. It identifies the notice by date and asks expressly for written confirmation that the employment continues.
Request for a copy of the employment contract and its amendments
You need it when applying for a loan, in a dispute about a post or about pay, or simply because your own copy is lost. The employer keeps the personnel file and issues certified copies of what is in it. The request lists the documents you are after by name, because a generally worded one usually comes back with a single sheet.
Application to work remotely
Remote work is not settled by a spoken arrangement — it is written into an amendment to the employment contract, which fixes the place of work, the working hours and who provides the equipment. This application puts the request in writing and proposes a specific arrangement, rather than leaving everything to be decided afresh the next time management changes.
Application for unpaid leave
Unpaid leave is not an entitlement — it is granted by the employer, so the application has to be in writing and has to carry a decision on it. State the exact period and the reason, because whether the time counts towards length of service and insurance depends on how long it is. File it early enough for someone to be able to cover your work.
Request for UP-2 and UP-3 certificates
These certificates are requested from an employer when retiring or when proving length of service: UP-2 shows insurable income, UP-3 the length and category of service. They are two different documents, issued separately, so mark which of them you need. If the employer no longer exists, they are sought from the regional archive rather than from the employer.
Objection to a disciplinary penalty
An order imposing a disciplinary penalty is challenged in writing and quickly — the time limit runs from its service, so the first thing to note down is the date you received it. The objection says specifically which of the alleged breaches you dispute and why: you were not heard, the act was not committed, the penalty does not match the gravity. Keep a copy with a reference number.
Application for leave on marriage, blood donation or bereavement
Paid leave under Art. 157 of the Labour Code is due on specific occasions — getting married, donating blood, the death of a close relative, attending court. It is separate from annual leave and is not deducted from it. The number of days is set by the Act and may be increased by a collective agreement, which is why it is a field here rather than an assumption.
Request for a reference from an employer
A reference is not a certificate: it is an assessment of your work, and an employer is not obliged to give one. So the request states which period, for what purpose and in which language you need it — a specific request is answered far more often than a general one.
Application to change the bank account for salary payments
Changing banks is a small matter until a salary goes to a closed account. This application tells the payroll office what the new account is and from which month it applies. File it before payday rather than after — the transfer is prepared in advance.
Notification to the employer of absence through illness
A sick note is issued by the doctor, but the employer has to be told that you are absent — at once, not when you come back. This document does that in writing: the date your absence starts, the number of the sick note, and how to reach you. The notification does not replace the sick note; it goes with it.
Request for a certificate of income
The bank, the court or the social services office wants a document showing your income, and the employer wants a written request. This form is that request: for which period, for what purpose, in how many copies. Stating the purpose matters — a certificate for a loan and a certificate for social assistance often carry different figures.
Application to terminate employment by mutual consent (Art. 325(1)(1) of the Labour Code)
The way to leave at once, without working a notice period — but only if the employer agrees. The application proposes a date on which the contract is to end. The part almost nobody knows: the employer must answer within seven days, and silence counts as a refusal. Without written consent there is no termination.
Notice of resignation (Art. 326 of the Labour Code)
When you leave of your own accord, the law wants written notice — not a conversation and not an email. This document puts it in the form Art. 326(1) of the Labour Code requires, states the notice period and names your last working day, so there is no argument about when the contract ends. The period is thirty days unless your contract says otherwise, and by law it cannot exceed three months.
