Find the document you need
Answer three short questions. You don't need to know what the form is called.
1. Which country is it for?
2. What is it about?
3. What do you want to do?

- Consent to act as managing director and specimen signatureThe document by which a managing director accepts the post and gives the specimen signature the Commercial Register will hold. Its whole point is the notarisation — the signature is given in front of a notary, not at home. The lower part of the sheet is left blank for exactly that; do not fill it in.
- Proxy for a general meeting of the owners' associationAn owner who cannot attend is represented by a proxy — but only on a written authority, which is attached to the minutes. Limit the powers if you do not want the proxy voting as they see fit: there is a separate field here for the items on which you are instructing them how to vote. A proxy that does not identify the meeting, or does not state the share of the common parts, is the one most often challenged.
- Consent to the processing of personal dataConsent is only one of the grounds for processing, and the most fragile one: it has to be specific, informed and capable of being withdrawn, and silence or a pre-ticked box is not consent. So this declaration lists the purposes separately and states the right to withdraw expressly. If you process data under a contract or under a statutory duty, you do not need consent and should not ask for it.
- Consent to being photographed and to the use of the imageNeeded by photographers, agencies, schools and event organisers — the image of an identifiable person is personal data, and using it for advertising or publication requires consent. The declaration sets out where and for how long the material will be used, whether there is a fee, and whether the person will be named. For a child it is signed by a parent.
- Power of attorney (general)The document by which you authorise someone else to do something in your place — to file an application, to collect a document, to represent you before an institution. You list the powers one by one and set a time limit. Note that for dealing with real property, with a vehicle, and in a number of proceedings before state bodies, the signature has to be notarised. We prepare the document; the notarisation is done at a notary.
- Owner's consent for address registrationWhen you register at an address that is not yours, the municipality asks for the owner's written consent. This is that document. It goes in together with the application for a permanent or current address, and without it the counter returns the whole file. If the owner will not be coming to the counter in person with their ID card, the signature has to be notarised — which is why most people end up going twice.
- Consent for a child to travel abroadWhen a child leaves the country with only one parent or with another adult, border control asks for the other parent's written consent. The declaration names the countries, the period and the accompanying adult by name. The signature has to be notarised — without that the document is of no use at the border. We prepare the wording; the notarisation is done at a notary.
