- Advance treatment directives – DAT, living will (Italy)For any adult who wants to state which medical treatments they accept or refuse, including artificial nutrition and hydration, in case they can one day no longer decide, and to name a trusted representative (fiduciario) (article 4 of Italian Law no. 219 of 22 December 2017). Many Comuni offer their own free model. The wishes are written by hand; the DAT are handed in person to the civil-status office of the Comune of residence, which forwards them to the national database: filing is free and needs no revenue stamp.
- Delegation to take a minor to medical appointments (Italy)For parents who cannot take their child to the paediatrician, a specialist visit, a test or vaccinations and ask a grandparent, babysitter or summer camp to do it. Many surgeries, local health authorities (ASL) and hospitals have their own free delegation form: if the provider asks for it, use theirs. This Italian document lets the carer accompany the child and receive the doctor’s instructions; consent to treatment stays with the parents. Both parents sign it.
- Proxy for a condominium meeting (Italy)For a flat owner in Italy who cannot attend the condominium meeting: the written proxy of art. 67 of the implementing provisions of the Italian Civil Code, naming the condominium, the unit, the date of the meeting and the person authorised to attend and vote, with any voting instructions. The proxy holder may not be the building's administrator and, in buildings with more than twenty owners, may not represent more than one fifth of the owners and of the value. The proxy holder hands it in at the start of the meeting. In Italian.
- Delegation to collect documents, certificates and registered letters (Italy)For anyone who cannot go in person and asks someone to run an errand: collecting a criminal-record certificate (casellario giudiziale) or another certificate, a registered letter held at the post office, a ready report or a document, or handing in a file. Poste Italiane and many offices have their own free delegation form: if the office asks for it, use theirs. This Italian delegation names the task, the office and the expiry date; the delegate brings their own ID and a copy of the delegator’s.
- Parents’ authorisation for a minor travelling with another adult (Italy)For parents who entrust their child to a grandparent, relative, teacher or sports group for a trip within Italy or abroad. A child under 14 travelling abroad without a parent needs the declaration of accompaniment (dichiarazione di accompagnamento) stamped by the Questura, which is free: this Italian letter does not replace it. The letter names the companion, the dates, the destination and the parents’ phone number; both parents sign it and attach copies of their ID.
- Special power of attorney for a single act (Italy)For anyone who cannot attend and asks a trusted person to carry out one act in their name: signing a contract, collecting a sum, representing them at a condominium meeting or before an office. The Italian power describes the act, its limits and its end date. If the act needs a special form – selling a house or a car, an act to be registered – a notarial power or a notarised signature is required. For a simple errand a delegation (delega) is enough.
- Appointment and acceptance of the trusted representative for advance directives (Italy)For anyone who has made advance treatment directives (DAT) without naming a fiduciario, or wants to replace them, and for the fiduciario who accepts by a separate act (article 4 of Italian Law no. 219 of 22 December 2017). The fiduciario represents the person with doctors and health facilities and receives a copy of the DAT. The act is handed, with the DAT or later, to the civil-status office of the Comune that received them, free of charge. Many Comuni have a free model.
- General power of attorney as a private writing (Italy)For anyone who wants a trusted person to handle their everyday affairs: paying bills and taxes, collecting money, picking up documents, dealing with the building’s condominium and with offices. Under Italian law a private power covers only ordinary administration and the acts expressly listed; selling or buying property, a gift or any act to be registered needs a notarial power. Banks and Poste Italiane may ask for their own forms. Signed by the principal and accepted by the agent.
