Council tax band proposal to the Assessor: statement of evidence (Scotland)
Scotland · in English
For the owner or council taxpayer of a home in Scotland who thinks it is in the wrong council tax band: a signed statement of your evidence — why the band is wrong, and the addresses and bands of similar homes nearby — to send or upload with your proposal to the local Assessor, who sets bands in Scotland. The Scottish Assessors' free online proposal form is the route; this statement carries your comparisons. A proposal can be made only in limited cases, most often within six months of your becoming the taxpayer or of the Assessor altering the band. The band can go up as well as down.
General power of attorney that ends on incapacity (Scotland)
Scotland · in English
For an adult in Scotland who wants someone they trust to handle their money and affairs while they can still decide for themselves — when abroad, in hospital or simply needing help. It ends if you become incapable: under section 18 of the Adults with Incapacity (Scotland) Act 2000, a power that is not a continuing or welfare power has no effect while you are incapable. For that, a continuing or welfare power of attorney is certified and registered with the Office of the Public Guardian (Scotland), whose guidance is the first place to look. Signed on paper before one witness.
Authority for a carer to consent to a child’s medical treatment (Scotland)
Scotland · in English · for parents
For a parent in Scotland leaving a child with grandparents, a friend, a childminder or another carer: a signed letter arranging for the carer to consent to the child’s medical treatment while the child is with them, and to speak to doctors and dentists. A parent may not give up parental responsibilities and rights but may arrange for another person to exercise them on their behalf (Children (Scotland) Act 1995, s 3(5)). You choose emergency treatment only, or routine care too, and the dates. The carer keeps the letter.
Letter of consent for a child to travel abroad (Northern Ireland)
Northern Ireland · in English · for parents
For a parent with parental responsibility who is not going on the trip: a signed letter consenting to the child travelling abroad, the Republic of Ireland included, with the other parent, grandparents, a school or another family. Taking a child under 16 out of the United Kingdom without everyone’s consent or the court’s leave can be an offence (Article 3 of the Child Abduction (Northern Ireland) Order 1985). There is no official form; the letter names the child, the trip and the dates.
Complaint about a package holiday that was not as booked (Scotland)
Scotland · in English · for consumers
For a traveller in Scotland whose package holiday — flights and hotel, or other travel services sold together — was not as booked: a different or poor hotel, missing services, a changed itinerary. Under the Package Travel and Linked Travel Arrangements Regulations 2018, which apply across the UK, the organiser is responsible for every part of the package and owes a price reduction and compensation for what went wrong. The letter claims them and is a letter before action with 14 days to reply, then simple procedure in the sheriff court. Send it to the organiser by email and by first class post.
Resignation letter giving notice to your employer (Northern Ireland)
Northern Ireland · in English · for employees
For an employee in Northern Ireland who is leaving their job: a letter that resigns, states the notice your contract requires (at least one week once you have worked a month, Article 118 of the Employment Rights (Northern Ireland) Order 1996) and the last day of employment, and asks for written confirmation, final pay including holiday earned and not taken, and your P45. Hand it to your manager and send it by email the same day, keeping a copy.
Annual leave request to your employer (Northern Ireland)
Northern Ireland · in English · for employees
For a worker in Northern Ireland who wants to book paid holiday: a dated written request giving the first and last day and the number of working days, asking for written approval. Unless your contract or holiday rules say otherwise, regulation 18 of the Working Time Regulations (Northern Ireland) 2016 asks for notice of twice the length of the leave, and lets the employer refuse only by counter-notice as long as the leave. The Labour Relations Agency explains the rules free. Send it by email or hand it in.
Formal complaint to your council (Scotland)
Scotland · in English
For a resident in Scotland unhappy with a council service or decision — bins, potholes, planning enforcement, noise, social work, council housing or poor handling of an earlier request: a formal complaint under the council's complaints handling procedure, saying what happened, what you have done so far and what you want the council to do. Use the council's online form if it has one. An investigated complaint is normally answered within 20 working days; if you are still unhappy after the council's final reply, the Scottish Public Services Ombudsman looks at complaints free of charge.
Request for a redetermination of a Social Security Scotland decision (Scotland)
Scotland · in English
For someone in Scotland who disagrees with a Social Security Scotland decision on Adult Disability Payment, Child Disability Payment, Carer Support Payment or another Scottish benefit. A written request for a redetermination under section 41 of the Social Security (Scotland) Act 2018 — within 42 days for those three benefits, later only with a good reason: it names the determination, says why it is wrong and lists new evidence; a redetermination comes before any tribunal appeal. You can also ask free by phone or online. Posted by recorded delivery. Sold blank, as it may hold health data.
Maternity leave notice to your employer (Northern Ireland)
Northern Ireland · in English · for employees
For a pregnant employee in Northern Ireland: the written notice of your pregnancy, the expected week of childbirth, the date you want your maternity leave to start and the date for Statutory Maternity Pay, given by the end of the 15th week before the expected week, with your MAT B1 certificate enclosed or to follow. It asks your employer to confirm in writing, within 28 days, when your leave ends and what you will be paid. nidirect explains maternity pay and leave free. Hand it in and email it.
Ask your landlord to return your deposit (Northern Ireland)
Northern Ireland · in English · for tenants
For a tenant in Northern Ireland who has moved out and is still waiting for the deposit. A letter before claim to the landlord or letting agent: it gives the date the tenancy ended, the deposit and the scheme protecting it — TDS Northern Ireland or myDeposits Northern Ireland — and asks for the deposit back within 14 days, or a written list of any deduction with evidence, saying which deductions you dispute. Next comes the scheme’s free dispute service, then a small claim in the county court (up to £5,000). If the deposit was never protected, report it to the council.
Complaint to your property factor (Scotland)
Scotland · in English
For a homeowner in Scotland whose property factor — the firm that manages the common parts of a tenement or development — charges unclear fees, neglects common repairs, mishandles the building insurance or does not answer. A formal complaint under the factor’s own complaints procedure: it sets out what went wrong, with dates, says why you think it breaks the factor’s duties or the Code of Conduct for Property Factors, and asks for the remedy you want. Notifying the factor in writing is required before a free application to the First-tier Tribunal for Scotland. Send it by recorded delivery or email, and keep a copy.
Council tax appeal to the council about liability or the amount charged (Scotland)
Scotland · in English
For a person in Scotland who disputes a council tax decision — that they are liable for a property, the amount charged, a refused discount or exemption, or that the property is chargeable at all: the written notice to the council that section 81(4) of the Local Government Finance Act 1992 requires before an appeal to the First-tier Tribunal for Scotland (Local Taxation Chamber). The council has two months to answer; the appeal must be made within four months of the first written notice you gave the council about the matter. Keep paying meanwhile. Sent first class.
Appeal against a dismissal or a disciplinary decision (Northern Ireland)
Northern Ireland · in English · for employees
For an employee in Northern Ireland who has been dismissed, warned or demoted after a disciplinary process: a letter appealing within your employer's time limit, setting out your grounds and what you ask for, and requesting an appeal meeting before a manager not involved in the decision, with a companion. For a dismissal it is the appeal step of the statutory dismissal procedure Northern Ireland keeps, and asks for the reasons in writing. Hand it in and email it.
Letter to Land & Property Services about your rates bill (Northern Ireland)
Northern Ireland · in English
For a ratepayer in Northern Ireland writing to Land & Property Services, which bills and collects domestic rates: to ask for Lone Pensioner Allowance (20% off for people aged 70 or over who live alone) or Disabled Person’s Allowance (25% off for an adapted home), to say your landlord is liable for a rented home valued at £150,000 or less, to dispute a wrong bill, or to ask for a refund. LPS has free application forms and online services; this letter suits a case they do not fit. To challenge the capital value itself, use the review statement instead.
Report disrepair, damp or mould and ask your landlord for repairs (Northern Ireland)
Northern Ireland · in English · for tenants
For a private tenant in Northern Ireland whose home needs repairs — damp and mould, a broken boiler, leaks, faulty electrics. The letter reports each problem in writing with dated photos, reminds the landlord of the repairing duties the Private Tenancies (Northern Ireland) Order 2006 sets where the tenancy agreement is silent and of the fitness standard, asks to arrange an inspection within 14 days, and confirms you will keep paying the rent. If nothing happens, the council’s environmental health team can inspect, free.
Request for term-time leave of absence for your child (Northern Ireland)
Northern Ireland · in English · for parents
For a parent in Northern Ireland who needs their child to miss school days in term time — a family wedding or funeral abroad, a serious illness in the family, a parent's leave from the forces. A request to the principal: it gives the dates and the day of return, explains the circumstances you believe are exceptional and asks for a written decision before the absence. The Department of Education's guidance lets a principal authorise term-time absence only in exceptional circumstances and no longer has a code for an agreed family holiday. Sent by email or handed in well in advance.
Letter telling a bank or organisation that a power of attorney has been revoked (Scotland)
Scotland · in English
For someone in Scotland who has revoked a general or one-off power of attorney: a letter to a bank, building society, utility, landlord or any organisation the attorney has dealt with, saying the power has been revoked, asking them to stop accepting the attorney’s instructions and to confirm it in writing. It encloses a copy of the signed revocation and goes by recorded delivery, or by hand. Not for a continuing or welfare power registered with the Office of the Public Guardian (Scotland), which is ended through the OPG.
Request to the council for a council tax discount or exemption (Scotland)
Scotland · in English
For a council taxpayer in Scotland: a letter to the council asking for the single person discount (25% off when you are the only adult at home), a discount because the other adults are disregarded, such as full-time students, or an exemption, from a date you give, with any backdating and refund. Every Scottish council has a free online form that does the same; this letter suits those who prefer writing or have a case the form does not fit. If the council refuses, the next step is a written appeal to it, then the Local Taxation Chamber.
Rent statement and receipt for rent paid (Northern Ireland)
Northern Ireland · in English · for landlords
For a private landlord or letting agent in Northern Ireland to confirm the rent a tenant has paid — for a benefit claim, a mortgage or visa application, a new landlord’s reference or the tenant’s own records. It names the property, the landlord registration number, the rent and how often it is paid, the period covered and each payment received, and states whether any arrears are owed, signed and dated. A landlord must give a free written receipt for rent paid in cash (Article 5 of the Private Tenancies (Northern Ireland) Order 2006).
Letter before a small claim to a company for an unpaid invoice (Northern Ireland)
Northern Ireland · in English · for creditors
For a business in Northern Ireland owed money by a company or LLP that has not paid an invoice. The final letter before a claim: it sets out the debt, claims statutory interest at 8% above the Bank of England reference rate and the fixed sum of £40, £70 or £100 under the Late Payment Act (or the interest your contract sets), gives 14 days to pay or dispute, and names the next step — a claim in the county court, heard as a small claim up to £5,000. It goes by Royal Mail Signed For or by hand.
Request for unpaid leave by agreement with your employer (Northern Ireland)
Northern Ireland · in English · for employees
For an employee in Northern Ireland who needs time off beyond their holiday — a long trip, a family matter, study — and wants their employer's agreement: a letter asking for unpaid leave between two dates, with the return date, and asking the employer to confirm in writing your return to the same job and what happens to holiday, pension and benefits. There is no general right to unpaid leave; parental leave and time off for dependants are separate statutory rights. Hand it in and email it.
Formal complaint to your council (Northern Ireland)
Northern Ireland · in English
For a resident of Northern Ireland unhappy with a council service or decision — bins and recycling, street cleaning, environmental health, planning, building control, leisure centres, council car parks, or poor handling of an earlier request: a formal complaint under the council's complaints procedure, saying what happened, what you have done so far and what you want the council to do. Use the council's online form if it has one. If you are still unhappy after its final reply, the Northern Ireland Public Services Ombudsman looks at complaints free of charge.

Within 30 days you can reject them for a full refund; after that, you ask for one repair or replacement first (Consumer Rights Act 2015, sections 9 to 23). The letter states the fault and your choice, and is your final letter before a claim: the trader has 14 days to reply before you make a small claim in the county court, which hears claims of up to £5,000. Send it to the retailer, not the manufacturer, by email and by recorded delivery (Royal Mail Signed For).
