School transfers, certificates, absences and learning plans in Italy
In Italy enrolment in the first year of state primary and secondary school is made online on the education ministry’s Unica platform, and enrolment in nursery school on the school’s paper form; in Valle d’Aosta and the provinces of Trento and Bolzano enrolment stays on paper. To change school, during the year too, the head of the current school must grant clearance (nulla osta, Article 4 of Royal Decree 653/1925). The request for clearance to transfer names the new school and carries both parents’ signatures where they share responsibility (Article 337-ter of the Italian Civil Code). Within compulsory schooling it needs no revenue stamp (Table B, Article 11, of Presidential Decree 642/1972).
With public offices a self-declaration is enough, and since 2020 it can also be used with private parties who accept it; for private use or abroad there is the request for a certificate of enrolment and attendance, with a space for the revenue stamp where the use is not exempt. A trip or a move is announced with the notice of absence for family reasons, giving the dates and asking for the work to catch up on. Most Italian regions have abolished the doctor’s note for a return after over five days; where the region or the school asks for a parent’s declaration, there is the declaration for returning after illness.
For a pupil with a specific learning disorder, the parent hands in the diagnosis and asks for a personalised learning plan (PDP) with the letter requesting the plan; the class council draws it up with the family within the first term (Law 170/2010; Ministerial Decree 5669/2011). The return declaration and the plan letter hold health data, so they are sold blank, to complete by hand. Many schools have their own free forms at the office or in the electronic register. The documents are written in Italian, are up to date on 13 October 2026 and are not official documents. DocMuse is a private paid service, and nobody at DocMuse reviews your situation.

Request for a certificate of school enrolment and attendance (Italy)
An Italian request, in Italian, from a parent or an adult student to the school office for a certificate of enrolment or attendance for private use: an employer, a bank, a sports body, a foreign authority. Many schools issue certificates through the electronic register; with public offices, and now with private parties too, a self-declaration is enough. It has the space for the 16 € revenue stamp (sold at tobacconists; @e.bollo online), which is not applied where the use is exempt. Hand it in at the school office or send it by PEC.
Parent’s declaration for returning to school after illness (Italy)
An Italian declaration, in Italian, by a parent or guardian for the school that the pupil was absent for health reasons between the dates given and that the parent followed the paediatrician’s or family doctor’s advice. It is for regions or schools that ask for it instead of a medical certificate, which most Italian regions have abolished. Many schools have their own form or a function in the electronic register, free of charge. It holds health data, so it is sold blank and filled in by hand.
Handing in a learning-disorder diagnosis and requesting a personalised learning plan (Italy)
An Italian letter, in Italian, from a parent or guardian to the head teacher handing in the diagnosis of a specific learning disorder (dyslexia, dysorthography, dysgraphia, dyscalculia) and asking for a personalised learning plan (PDP) with compensatory tools and dispensatory measures (Italian law 170/2010), and to take part in drafting it. Many schools have their own free form. Hand it in at the school office against a registration number or send it by PEC. It holds health data, so it is sold blank and filled in by hand.
Notice of a pupil’s absence for family reasons (Italy)
An Italian letter, in Italian, from a parent or guardian to the head teacher and the class coordinator announcing in advance, or justifying afterwards, a pupil’s absence for family reasons (a trip, a move, a family event), with the dates, and asking what homework and topics to catch up on. For short absences many schools use the electronic register or the absence booklet, which are free. It is not for absences through illness. Hand it in at the school office, send it by PEC or upload it to the register where the school allows.
Request for clearance to transfer to another school (Italy)
An Italian letter, in Italian, from a parent or guardian to the head of the pupil’s current school asking for the nulla osta (clearance) to transfer the pupil to another school, also during the school year, and for the school records to be sent on. It gives the pupil, the class, the new school and the reason, with the other parent’s signature where parental responsibility is shared. Many schools have their own free form at the office or online. In compulsory schooling no stamp duty is due (Italian Presidential Decree 642/1972, table B, art. 11). Hand it in at the school office or send it by PEC.
