This document is in Italian

The PDF you download is in Italian — that is the language it has to be filed in, so it is not translated. Everything on this page is here to tell you what it says.

An Italian declaration, in Italian, by a parent or guardian for the school that the pupil was absent for health reasons between the dates given and that the parent followed the paediatrician’s or family doctor’s advice. It is for regions or schools that ask for it instead of a medical certificate, which most Italian regions have abolished. Many schools have their own form or a function in the electronic register, free of charge. It holds health data, so it is sold blank and filled in by hand.

Common questions

After five days of illness, is a medical certificate still needed to return to school in Italy?
It depends on the region: most Italian regions have abolished the readmission certificate by their own laws, and some, or some schools in their rules, ask instead for a parent’s declaration. Check the rule of your region and the school’s rules. Many schools have their own form or a function in the electronic register, free of charge: use them where they exist.
Who signs the declaration, and how is it handed in?
A parent or guardian signs it; for an adult student the school may accept it from the student. Hand it to the first-period teacher or the school office on the day of return, or upload it to the electronic register if the school provides for that. Keep a copy or a photo of the sheet you handed in.
Why is the return-to-school declaration filled in by hand?
It says that a child was ill and for how many days: that is health data, which Regulation (EU) 2016/679 protects specially. That is why DocMuse sells it blank, to print and complete in pen, and collects none of this data online. There is no need to name the illness: the period of absence is enough.

  • Request for clearance to transfer to another school (Italy)

An Italian letter, in Italian, from a parent or guardian to the head of the pupil’s current school asking for the nulla osta (clearance) to transfer the pupil to another school, also during the school year, and for the school records to be sent on. It gives the pupil, the class, the new school and the reason, with the other parent’s signature where parental responsibility is shared. Many schools have their own free form at the office or online. In compulsory schooling no stamp duty is due (Italian Presidential Decree 642/1972, table B, art. 11). Hand it in at the school office or send it by PEC.

  • Request for a certificate of school enrolment and attendance (Italy)

An Italian request, in Italian, from a parent or an adult student to the school office for a certificate of enrolment or attendance for private use: an employer, a bank, a sports body, a foreign authority. Many schools issue certificates through the electronic register; with public offices, and now with private parties too, a self-declaration is enough. It has the space for the 16 € revenue stamp (sold at tobacconists; @e.bollo online), which is not applied where the use is exempt. Hand it in at the school office or send it by PEC.

  • Handing in a learning-disorder diagnosis and requesting a personalised learning plan (Italy)

An Italian letter, in Italian, from a parent or guardian to the head teacher handing in the diagnosis of a specific learning disorder (dyslexia, dysorthography, dysgraphia, dyscalculia) and asking for a personalised learning plan (PDP) with compensatory tools and dispensatory measures (Italian law 170/2010), and to take part in drafting it. Many schools have their own free form. Hand it in at the school office against a registration number or send it by PEC. It holds health data, so it is sold blank and filled in by hand.

  • Notice of a pupil’s absence for family reasons (Italy)

An Italian letter, in Italian, from a parent or guardian to the head teacher and the class coordinator announcing in advance, or justifying afterwards, a pupil’s absence for family reasons (a trip, a move, a family event), with the dates, and asking what homework and topics to catch up on. For short absences many schools use the electronic register or the absence booklet, which are free. It is not for absences through illness. Hand it in at the school office, send it by PEC or upload it to the register where the school allows.
