A report goes further than a phone call, because it is logged with a reference number and has to be answered. Describe the location as precisely as you can and say whether anyone is in danger — that is what determines how quickly it is dealt with.

Why write a report when I can just phone?

Because a written report is logged with a reference number and has to be answered, whereas a phone call leaves no trace. If the problem recurs or causes damage, a dated report shows when the municipality was told.

Will I get an answer?

As a rule yes, to the address or email given, which is why both are fields on the form. An absence of any answer within a reasonable period is itself a ground for a next step.

Can I report something anonymously?

You can, but an anonymous report usually cannot be answered and is dealt with at lower priority. If you want a result and to be kept informed, sign it.

How should I describe the location?

As precisely as you can — address, junction, landmark, the number on a lamp post or manhole. A report of a pothole on a long street with no landmark often comes back saying no problem was found.
