Receipt for documents handed over
For every time you leave originals with someone else — an accountant, a lawyer, an employer, a bank, an agent. The receipt lists the documents one by one, marks which are originals and which copies, and gives a date for their return. An original handed over without a receipt is later chased with no evidence that it was ever handed over at all.
Contract of mandate
For when you instruct someone to act on your behalf — to represent you before an institution, to manage a property, to negotiate, to receive payments. It differs from a contract for work and from a service agreement in that the agent acts for your account and owes you an account of it. The contract settles the scope, the reporting and the expenses; for acting towards third parties a separate power of attorney is also needed.
Declaration of family and financial circumstances
Asked for when applying for social assistance, legal aid, a grant, a reduced fee or municipal housing. Many institutions have a form of their own — if there is one, use it; this document is for when there is not, or when you want to prepare the figures in advance. It is declared under liability, so set out your income in full.
Record of an item handed in for repair
You are leaving a phone, a laptop, an appliance or a machine at a repair shop. The record describes the item, the visible damage at the point of handover and the fault reported — and fixes whether a deadline and an estimated price were given. It is also the document a later complaint about a poor repair rests on, because it shows the condition the item was in when it went in.
Deed of gift for movable property
A gift between people close to one another is ordinary enough, but an undocumented gift creates two problems: there is later no proof of whose the thing is, and no proof that it was not a sale. The contract fixes who gives what to whom and from which date. A gift between lineal relatives and between spouses is normally exempt from tax; in every other case the municipality charges local tax, so check before signing.
Consent to being photographed and to the use of the image
Needed by photographers, agencies, schools and event organisers — the image of an identifiable person is personal data, and using it for advertising or publication requires consent. The declaration sets out where and for how long the material will be used, whether there is a fee, and whether the person will be named. For a child it is signed by a parent.
Loan for use
For when you lend out a thing free of charge — a tool, a piece of equipment, a car, a room. It differs from a lease in that nothing is paid, and from a money loan in that the same thing comes back rather than its value. So the description of the item and its condition at handover carry the whole weight of the contract: without them there is no way to show that what came back is not what went out.
Proxy for a general meeting of the owners' association
An owner who cannot attend is represented by a proxy — but only on a written authority, which is attached to the minutes. Limit the powers if you do not want the proxy voting as they see fit: there is a separate field here for the items on which you are instructing them how to vote. A proxy that does not identify the meeting, or does not state the share of the common parts, is the one most often challenged.
Consent to the processing of personal data
Consent is only one of the grounds for processing, and the most fragile one: it has to be specific, informed and capable of being withdrawn, and silence or a pre-ticked box is not consent. So this declaration lists the purposes separately and states the right to withdraw expressly. If you process data under a contract or under a statutory duty, you do not need consent and should not ask for it.
Application for childcare leave
Leave under Art. 164 of the Labour Code is taken after maternity leave and is granted on an application to the employer, while the benefit is paid by the social security institute through a separate procedure. The age limit and the amount of the benefit are set by law and do change — check them for the year you are applying in. The application also states whether the leave is being transferred to the other parent or to a grandparent, which is common.
Request for a copy of medical records
A patient is entitled to a copy of the records of their own treatment: discharge summaries, test results, imaging, outpatient notes. You need it for a second opinion, when changing doctor, for an insurance claim, or before a medical board. State the period and the kind of documents, because a general request is usually met with a single discharge summary.
Request to convene a general meeting of the owners' association
When the manager does not call a meeting, the owners can ask for one in writing. The request has to carry the signatures of enough owners and to set out a ready agenda — a meeting cannot decide on items that were not announced. If the manager does not call the meeting in time, those who asked for it may call it themselves.
Report of a problem to the municipality
For street lighting that does not work, potholes, an abandoned vehicle, illegal dumping, noise, a dangerous tree, a missing manhole cover. A report goes further than a phone call, because it is logged with a reference number and has to be answered. Describe the location as precisely as you can and say whether anyone is in danger — that is what determines how quickly it is dealt with.
Minutes of a general meeting of the owners' association
Decisions of a general meeting can only be proved by the minutes, and minutes with no quorum, no announced agenda or no recorded votes are successfully challenged. So each of those is a separate field here. The Condominium Management Act sets requirements for both the calling of the meeting and the content of the minutes — check them before the meeting, because afterwards they cannot be put right.
Application for reconsideration of a decision
The general document for when an institution has refused something and you believe the refusal rests on incomplete or mistaken information. The application identifies the decision by number and date, says specifically what in it you dispute, and sets out the new fact or document you are putting forward. It does not replace an appeal and does not stop the time limits for one — if there is a deadline to appeal, watch it separately.
Notice to a neighbour of damage caused
A leak from above is the commonest dispute between neighbours and is almost always conducted from memory, because nobody wrote anything down. This notice dates the event, describes the damage and invites the neighbour to a joint inspection — and a refusal to attend is itself a fact that matters later. If an insurer is involved, written notice is the first thing they will ask you for.
Report to the manager of the owners' association
For leaks in the common parts, broken lighting, a lift out of service, mess, or persistent breaches of the house rules. The difference between a grumble and a report is that a report is dated and describes the problem specifically — since when, where exactly, and what damage it is causing. The manager's duty to act starts running from its receipt, not from the first complaint on the stairs.
Application for exemption from physical education
Exemption is granted by the head teacher and almost always rests on a medical document — an application with no certificate attached is usually sent back. Mark whether you are asking for a full exemption or only from particular activities, because a partial one is granted more readily. The pupil still has to attend the lesson unless the decision says otherwise.
Application to transfer to another course or mode of study
A transfer depends on available places and on how many of the examinations already passed will be recognised — which is why this application expressly asks for recognition of the subjects and a plan for making up the difference. Without that question a student transfers and then discovers they owe ten examinations. Moving from full-time to part-time study also changes the fee and the entitlement to a hall place and a grant.
Application to reduce or waive a fee
The grounds differ from one school and university to the next — financial circumstances, outstanding results, being orphaned, a disability, a large family — but the procedure is the same: a written application with documents attached, filed in time and before the fee is paid. Once the fee has been paid the request becomes a claim for a refund, which is slower and refused more often.
Application to sit a resit examination
Sitting a resit is applied for in advance and within the deadline — being entered on the examination record follows from an application, not from turning up in the hall. Give the subject, the examiner and the session exactly as they appear in the curriculum, because a mismatch is the commonest reason an application comes back. Check too whether a fee is payable.
Application for a duplicate diploma
Where a diploma is lost, destroyed or damaged, a duplicate is issued from the register held by the school or university. The application identifies exactly which diploma is wanted — year of completion, course, registration number if known — because the issuer searches an archive rather than a name. A fee is usually payable and a declaration of the circumstances required.
Application to interrupt an academic year
Interrupting studies is granted on particular grounds — health, family, work abroad, maternity — and almost always requires a document. What the application asks expressly is which examinations remain outstanding and how they are to be made up, because on returning a student is reinstated into a specific year, not where they remember being.
Application for after-school provision
A place in after-school provision is at the parent's request and is applied for in writing, usually before the school year starts, because whether a group is formed at all depends on how many applications come in. The application states from which date, until what time, and who will be collecting the child — the last matters, because a teacher will not release a child to anyone not on the list.
