Common questions

The institution has its own form. Should I use this one?
If there is an official form, use it — this document is for when there is not, or when you want to prepare the figures in advance. A declaration prepared beforehand saves a second trip over a box left blank.
Who counts as a member of the household?
Usually the people who live together and share a budget, whatever the family relationship. The definition differs between schemes, so check how it is defined for the particular benefit before filling this in.
Do I have to state income I have no document for?
Yes. The declaration is for the full picture, not only for what is documented. Income left out and discovered later is treated as a false declaration, with the consequences the declaration expressly sets out.
What if my circumstances change?
You are obliged to tell them. Assistance granted once the grounds for it have gone is repayable, and an unreported change makes the case worse. That is why the declaration contains an express undertaking to notify.
