- Application for care allowance – Pflegegeld (Austria)The application for care allowance (Pflegegeld), or for a higher level of it, to the PVA, SVS, BVAEB or AUVA under Austria's Federal Care Allowance Act (BPGG) — with insurance number, pension received, treating doctor, the help needed in your own words, a person you trust for the assessment, and IBAN. Care allowance is due from the first of the month after the application arrives (§ 9 BPGG): the day it arrives counts. The insurers provide their own form free of charge, but an application without it is valid too. In German, to print, sign and send promptly.
- Request for information under the Freedom of Information Act (Austria)The written request for information under Austria's Freedom of Information Act (IFG), which since 1 September 2025 has replaced official secrecy and the old duty-to-inform acts: to an authority of the federation, a Land or a municipality, or to a company audited by the Court of Audit. With the exact information you want, the form you want it in, and a reminder of the four-week deadline (§ 8 IFG). The request is free. If an authority refuses the information, the next step is the request for a formal decision — a separate document; against a company, an application goes straight to the administrative court. In German.
- Claim for reimbursement to a private health insurer (Austria)The letter for sending doctor's, therapy or hospital bills to your private health insurer in Austria for reimbursement — under a non-contracted-doctor or private-ward (Sonderklasse) tariff, for instance: with policy number, patient, treatment, invoices, the amount already refunded by the statutory insurer (ÖGK, SVS or BVAEB) and IBAN, with no diagnosis in the letter. If the insurer refuses, it asks for written reasons naming the policy clause relied on. Many insurers offer an app or customer portal for uploading invoices; your policy terms say which documents are needed. In German.
- Request to refer a tax complaint to the Federal Fiscal Court (Austria)The request (Vorlageantrag) to have your complaint decided by Austria's Federal Fiscal Court (Bundesfinanzgericht) when the tax office's preliminary decision on it (Beschwerdevorentscheidung) does not settle the matter, under § 264 of the Austrian Federal Fiscal Code (BAO): within one month of service, lodged with the tax office. It names the preliminary decision, can add grounds, and is the last chance to ask for an oral hearing and a panel (§§ 272, 274 BAO); a fresh application to suspend collection is optional. Through FinanzOnline, by post or fax: an e-mail is no submission at all (§§ 85, 86a BAO). In German.
- Tenant's application to the conciliation board or district court under § 37 MRG (Austria)The tenant's application in Austria's non-contentious tenancy procedure: to have the main rent checked, the deposit repaid, the service-charge statement reviewed or repairs carried out (§ 37 Abs 1 of the Austrian Tenancy Act, MRG) — with the parties, the flat, the request, the grounds and the enclosures. Where the municipality has a conciliation board (Schlichtungsstelle), it is approached first (§ 39 MRG); elsewhere the district court for the flat. Vienna prescribes no form; if your municipality offers its own, you may use it free of charge instead. Signed by all tenants. In German.
- Subletting a flat: request for the landlord's consent, or notice to the landlord (Austria)The letter to the landlord when you sublet part of a rented flat in Austria: a request for consent where the lease requires it, or a notice where the lease does not forbid subletting. It names the rooms, the period, the subtenant, the sublet rent and the number of occupants, and relies on § 11 of the Austrian Tenancy Act (MRG) — where a ban binds only for an important reason — or on § 1098 of the Austrian Civil Code (ABGB). It does not cover short lets to changing guests through platforms. In German.
- Request for exam adjustments at school for a disability (Austria)The request to an Austrian school's head to take a pupil's disability or health condition into account in class tests (Schularbeiten), tests and exams — with more time, a separate room or an oral instead of a written form, say — without lowering the curriculum's standard. It rests on § 18 Abs 6 of the School Education Act (SchUG), applies in every Land and describes the effects, not the diagnosis; a medical or specialist report is enclosed. The Matura and dyslexia have rules of their own. For parents and adult pupils. In German.
- Request for a certificate of school attendance (Austria)The written request to an Austrian school's office for a certificate of attendance (Schulbesuchsbestätigung) — for instance for the family allowance (Familienbeihilfe) of an adult child still in education, for which the Finanzamt Österreich asks for proof, or for an office abroad. It gives the pupil's name and date of birth, class and school year, who the certificate is for, the number of copies and the date it is needed by. For parents of pupils under 18 and for adult pupils themselves, for use in every Land. In German, ready to print, sign and hand in.
- Application for a different exam method at university (Austria)The application by a student with a disability for a different exam method at an Austrian university — an oral instead of a written exam, more time or a separate room, for instance. Under § 59 Abs 1 Z 12 of Austria's Universities Act (UG) there is a right to it where the disability makes the prescribed method impossible and the exam's content and standard stay the same. With student number, programme, exams, effects, the method asked for and evidence. Apply in good time; if your university has its own free form, as the University of Vienna does, use that. In German.
- Request for early admission to primary school (Austria)The parents' written request to the head of an Austrian primary school (Volksschule) to admit a child not yet of compulsory-school age to the first year early — for children who turn six by 1 March of the following calendar year (§ 7 of the Compulsory Schooling Act, SchPflG, the same in every Land). It is made within the enrolment period; the school checks readiness for school and social skills by meeting the child and through a school doctor's report. With a choice on a school psychologist's report. Enrolment itself is in person; if the school has its own form, use that. In German.
- Request to see the medical record and for a copy (Austria)The letter to a doctor's practice (Ordination) or hospital in Austria asking to see your medical record (ärztliche Dokumentation, Krankengeschichte) and for a complete copy — findings, doctors' letters, imaging, surgery and nursing reports. Since 30 July 2026 the first copy is free under the Austrian Physicians Act (§ 51 Abs 1 ÄrzteG 1998); for hospitals the letter relies on the Hospitals Act (§ 10 KAKuG), the Land's hospital act and Art 15 GDPR, under which the first copy is free too. Deadline: one month (Art 12(3) GDPR). You choose a copy, or inspection and a copy, and e-mail, post or collection. In German.
- Application to determine insurance periods (Austria)The application to the PVA, SVS or BVAEB to determine your insurance periods bindingly, by formal decision (Bescheid), under § 247 of Austria's General Social Insurance Act (ASVG), or § 117a GSVG for those insured with the SVS — the way to settle missing periods such as child-raising, military or civilian service or employment before claiming a pension, rather than relying on the non-binding account statement. With insurance number, date of birth, former surnames and the periods at issue; proof is enclosed. The decision can be challenged at the Labour and Social Court. In German.
- Conciliation request in a dispute with a bank (Austria)Your application to the joint conciliation body of the Austrian banking industry (Gemeinsame Schlichtungsstelle der Österreichischen Kreditwirtschaft) in Vienna, when your bank has rejected your complaint or not answered it — over transfers, card payments, charges, an account or other banking business. It names the bank, the contract, the complaint and the answer, describes the dispute and the amount claimed, and contains the usual declarations, including releasing the bank from banking secrecy. The procedure is free for you and voluntary for the bank; the body proposes a solution. Foreign-currency loan disputes go to the consumer conciliation body. In German.
- Application for conciliation at E-Control – electricity and gas (Austria)E-Control, Austria's energy regulator, offers its own free online form for its conciliation body. This document is your written application — to send by post or email, or as ready text to paste into that form: the company, metering point, the facts, your earlier complaint and the answer, what you ask for and the amount in dispute, with your consent to its being passed to the company. For disputes with electricity or gas companies about bills, switching or disconnection (§ 26 E-ControlG). Free; the companies must take part. In German.
- Application to set aside a tax assessment under § 299 BAO (Austria)The application to Austria's tax office, the Finanzamt Österreich, to set aside a tax assessment whose ruling is wrong, under § 299 of the Austrian Federal Fiscal Code (BAO) — the route once the one-month deadline for a complaint has passed. It must be lodged within one year of notification of the assessment (§ 302 BAO), names the assessment and says why it is wrong; a new assessment is issued together with the setting aside. Whether to set it aside is at the tax office's discretion. Through FinanzOnline, by post or fax: an e-mail is no submission at all (§§ 85, 86a BAO). In German.
- Request to extend the deadline for a tax complaint (Austria)The request to Austria's tax office, the Finanzamt Österreich, to extend the one-month deadline for a complaint against a tax assessment (§ 245 Abs 3 of the Austrian Federal Fiscal Code, BAO) — when documents are missing or there is not enough time for the grounds. It must be lodged, with reasons, before the deadline ends, and may be repeated; it halts the deadline until it is decided, but never beyond the day requested (§ 245 Abs 4 BAO). With the assessment, the date of service, the current end of the deadline and the day sought. Through FinanzOnline, by post or fax: an e-mail is no submission at all (§§ 85, 86a BAO). In German.
- Request for a copy or duplicate of a school report (Austria)The written request to an Austrian school for a copy or a duplicate of a school report — the annual report, a leaving certificate or the Matura certificate (Reifeprüfungszeugnis) — when the original is lost, damaged or destroyed or another copy is needed. It gives the report, the school year, the years at the school, the date of birth and the name used at school, so the school can find its records. The school will tell you whether there is a charge; the letter asks, and asks who holds the records if the school no longer does. In German.
- Request for leave from lessons at a school (Austria)The written request by which parents — or pupils of full age themselves — ask an Austrian school in advance to let a pupil stay away from lessons, for instance for a wedding or funeral in the family, therapy or a competition. For children of compulsory-school age, leave is granted only as far as strictly necessary: single lessons up to a day by the class teacher (Klassenvorstand), up to a week by the head, longer by the Land's education directorate (§ 9 Abs 6 SchPflG); past compulsory schooling, beyond a day by the head (§ 45 Abs 4 SchUG). Many schools post their own form free on their website — if yours requires it, use that. In German.
- Application to change a surname or first name (Austria)Only for Austrian citizens, and for stateless people and refugees living in Austria: the written application for a change of name under Austria's Name Change Act (NÄG), to the district authority (Bezirkshauptmannschaft) or Magistrat where you live — in Vienna, municipal department MA 63: for the surname, the first name or both, with your details, the name you want and your reasons. In Austria, simply wishing for another surname is a ground of its own (§ 2(1) no. 11 NÄG). Returning to an earlier surname after a divorce is instead a declaration at the registry office. In German, ready to print and sign.
- Application to protect a garnished bank account (Austria)Your application to the enforcement court in Austria when your bank account has been garnished and your wages, salary, pension or other income that is only partly garnishable is paid into it. Austria has no special protected account: on your application the district court lifts the garnishment under § 292i EO as far as the balance matches the protected part of your income until the next pay date, and if asked, in advance for what you urgently need. With the case number, bank, IBAN, income and pay dates. By post, in person or through JustizOnline — not by e-mail; you can also apply orally at the court. In German.
- Claim for reimbursement after a non-contracted doctor or therapist (Austria)The covering letter for sending paid fee notes from a Wahlarzt or Wahltherapeut — a doctor or therapist without a contract with the insurer — to Austria's statutory health insurers ÖGK, SVS or BVAEB: with insurance number, patient, treatment period, fee notes and IBAN, and for the psychotherapy subsidy too. The ÖGK refunds 80 % of its contract tariff, not of the bill (§ 131 ASVG, Austria's General Social Insurance Act), if the claim arrives within 42 months of treatment (§ 102 ASVG). Online through MeineSV or the insurer's app is free and usually simpler; this letter is for claims on paper. In German.
- Application for conciliation at the Verbraucherschlichtung Austria or the Internet Ombudsstelle (Austria)Both conciliation bodies offer their own free online form. This document is your written application — to send, or as ready text to paste into the form: the trader, the contract, the facts, your earlier complaint and the answer, what you ask for and the amount, with the declarations a conciliation body needs. The Internet Ombudsstelle handles contracts made online, the Verbraucherschlichtung Austria all other consumer contracts. Free; taking part is voluntary for the trader (Austria's Alternative Dispute Resolution Act, AStG). In German.
- Request to extend the deadline for filing a tax return (Austria)The request to Austria's tax office, the Finanzamt Österreich, to extend the deadline for an income tax, VAT or other tax return under § 134 Abs 2 of the Austrian Federal Fiscal Code (BAO): with the return, the year, the current and the requested deadline, and the reasons. It must be filed before the deadline runs out; if it is refused, the office sets a further period of at least one week. Missing the deadline without excuse risks a late-filing surcharge of up to 10% (§ 135 BAO). By post, fax or FinanzOnline: an e-mail is no submission at all (§§ 85, 86a BAO). In German.
- Request to attend a school outside the catchment area (Austria)The parents' request for their child to attend a public compulsory school — a Volksschule or Mittelschule, for instance — outside the catchment area (Schulsprengel) the child belongs to. The federal framework is § 13 of Austria's Compulsory Schools Maintenance Framework Act; each Land makes the rules: in some Länder the municipality of residence decides, the consent of the body maintaining the chosen school is required, and the deadline falls early in the year. With the Land, the child, the main residence, the chosen school, the reasons and the consent. In German, ready to print and sign.
