- Annual leave request (Cyprus)An employee's written request to the employer in Cyprus for paid annual leave: dates, working days, the leave year and who covers, with room for the employer's approval on the same sheet. Under the Annual Leave with Pay Law of 1967, a full year's work earns at least 20 working days on a five-day week and 24 on a six-day week; a contract may give more. A written request leaves a record of what was asked for and what was approved.
- Invitation letter for a visa (Cyprus)The letter in which a resident of Cyprus invites a relative or friend from a country that needs a visa, addressed to the Cypriot embassy or consulate: the host's and the guest's details, the purpose and dates of the visit, where the guest will stay and the costs the host takes on. It does not replace the official assumption of responsibility for hosting form, signed with a certified signature and backed by a bank guarantee; the letter mentions that form and states that it will be submitted if asked for.
- Parental leave request to the employer (Cyprus)A working parent's written request to the employer in Cyprus for parental leave under Law 216(I)/2022: the child, the dates, the length and the leave already taken, with room for the employer's reply. After six months' service the Law gives up to 18 weeks for each child until the child turns 15 (21 with a disability), from one day to five weeks a year (seven with three or more children), on a request made at least three weeks before the start. Social Insurance Services pay a benefit for part of the leave, eight weeks for the first child and more for later children, on the parent's own separate claim.
- Request for a certificate of employment (Cyprus)A written request by an employee or former employee in Cyprus asking the employer for a certificate of employment: the job and the period worked, and, if wanted, the pay or an assessment of the work. It serves a new employer, a bank, a landlord, a residence permit or studies. A written request, with a date for the reply, leaves proof of what was asked for and when.
- Request for access to public-sector information (Cyprus)The request by which anyone asks a Cypriot ministry, department or other public authority for information and documents it holds, under Law 184(I)/2017 on the right of access to public-sector information. It describes what is asked for and how, with no reasons, which the Law does not require, and asks for a written answer within the Law's period, notice of any fee before access is given, partial access where part of the information is exempt, and the reasons for any refusal, which open the way to a complaint to the Information Commissioner.
- Request for a social insurance record statement (Cyprus)The written request to the Social Insurance Services in Cyprus for a statement of one's insurance record: periods of insurance, insurable earnings, contributions and employers for the period you choose. It serves to check that an employer paid the contributions, before a claim for a benefit or pension, or for use in another country. It is also made as a request for access to personal data under art. 15 of the General Data Protection Regulation. With a verified CY Login account the statement can also be seen on gov.cy; this letter is for asking in writing, at the district office, a Citizen Service Centre or by email.
- Request for access to a medical record (Cyprus)The written request in which a patient in Cyprus asks a hospital, clinic or doctor for access to their medical record and a copy of it: history, discharge note, tests, imaging. It rests on the Safeguarding and Protection of Patients' Rights Law (Law 1(I)/2005) and art. 15 of the General Data Protection Regulation. It names the department, the period of treatment, the documents wanted and how to receive them. The first copy is free, and the answer is due within one month, extendable by two months only for complex or numerous requests.
- Claim under private health insurance (Cyprus)The letter in which an insured person in Cyprus notifies a private insurer of a hospital stay, an operation or other treatment and claims the benefit the health policy provides: the policy, the treatment, the provider and dates, the amount in figures and words, the IBAN and the attachments. It states whether part of the treatment was covered by GESY, the national health system, or by other insurance. It is addressed to a private insurer, not to GESY, whose services are provided through its contracted providers.
- Request for a certificate of studies (Cyprus)The written request to a school in Cyprus for a copy of the leaving certificate (apolytirio), a certificate of studies with the marks for each year, or a certificate of attendance: the graduate's details as the school keeps them, the name while at school if it has changed, the year of graduation, the number of copies and the purpose. If the records are now kept elsewhere, it asks for the request to be passed on to whoever keeps them. It serves for a job, a university application or recognition abroad.
- Request for a tax clearance certificate (Cyprus)A request to Cyprus's Tax Department for a tax clearance certificate, asked for, for example, when bidding in a public tender or renewing a work permit. It gives the purpose and the date the certificate is needed by, states whether the tax returns have been filed and whether any tax is owed or paid in agreed instalments, and asks for any outstanding item to be notified in writing. The Department takes such requests by e-mail to its district offices; for VAT, the certificate is issued only through Tax For All (TFA).
- Request to pay overdue tax in instalments (Cyprus)A request to Cyprus's Tax Department to pay overdue tax in monthly instalments: the debt, the number and amount of instalments you propose, the first payment and the reasons. It states whether every tax return has been filed, commits to paying current taxes on time, and asks for the exact amount owed and written confirmation of the schedule. gov.cy's page on instalment plans directs new plans to the Department by e-mail, so the document can also be sent as an attachment.
