The PDF you download is in Greek — that is the language it has to be filed in, so it is not translated. Everything on this page is here to tell you what it says.

The written request to a school in Cyprus for a copy of the leaving certificate (apolytirio), a certificate of studies with the marks for each year, or a certificate of attendance: the graduate's details as the school keeps them, the name while at school if it has changed, the year of graduation, the number of copies and the purpose. If the records are now kept elsewhere, it asks for the request to be passed on to whoever keeps them. It serves for a job, a university application or recognition abroad.

Common questions

Where is a copy of a leaving certificate or a certificate of studies requested in Cyprus?
From the school you graduated from, which issues it from its records, by a written request with a copy of your ID. For a private school that has stopped operating, the certificate of attendance or graduation is requested from the Ministry of Education, Sport and Youth on its own application form, which this letter does not replace. If the records are now kept by another school or office, the request asks for it to be passed on there and for the applicant to be told.
Can a former pupil ask a school in Cyprus for the personal data it holds on their schooling?
Yes. Under art. 15 of the General Data Protection Regulation (Regulation (EU) 2016/679), a former pupil has a right of access to the data the school holds on them, such as marks and the school years attended, and to a copy, the first free of charge. The answer is due within one month, extendable by two months for complex or numerous requests (art. 12(3)). A copy of the data is not a certificate of studies: the school issues the certificate on a request such as this one.
What is needed to use a Cypriot certificate of studies abroad?
For a country party to the 1961 Hague Convention, an apostille, which for Cypriot public documents is issued by the Ministry of Justice and Public Order for a €5 fee, and an official translation if the country where it will be used asks for one. Before the apostille, a secondary school leaving certificate must be certified by the Ministry of Education, Sport and Youth; if it is not, the competent Directorate of Secondary General or Secondary Technical and Vocational Education and Training, or the District Office of Secondary Education, certifies it. That is why the request states the purpose and asks for as many copies as needed.

The note in which a parent or guardian of a pupil at a school in Cyprus justifies the child's absence in writing: the dates, the class, the reason (illness, a medical appointment or a serious family reason) and whether a doctor's certificate is attached. In a primary school, more than five days' absence for health reasons needs a doctor's certificate unless the head is satisfied otherwise. In a gymnasium or lyceum, the note is handed in on the pupil's return and within five working days at the latest, two in May and June.
