The PDF you download is in German — that is the language it has to be filed in, so it is not translated. Everything on this page is here to tell you what it says.

The letter to a school office in Germany by which parents, or pupils who are of age, ask for a certificate of attendance (Schulbescheinigung) — the confirmation that a child attends the school, as needed for example for child benefit for a child over 18, for education grants or for a pupil's travel pass. It gives the pupil's name, date of birth, class and school year, the office the certificate is for, the number of copies and the date it is needed by, and asks the school to complete that office's own form if one is enclosed. In German, ready to print, sign and hand in.

Common questions

What if the German child benefit or education grant office has its own form for the school?
Then the school fills in that form. You get it free from the office that asks for it; enclose it with this letter. The letter expressly asks the school to confirm the details on an enclosed form — it does not replace that form, it goes with it.
Who may ask a German school for a certificate of attendance?
The parents, for a child who is a minor; pupils who are of age ask for it themselves. A school does not give out information about its pupils to just anyone, which is why the letter gives the name, date of birth and class and is signed. School law is Land law: what the certificate looks like and whether the school also issues it electronically differ from Land to Land and from school to school.
What is the difference between a German certificate of attendance and a school report?
The certificate of attendance only confirms that someone attends the school at a given time, usually with the class and the expected end of schooling. A report (Zeugnis) assesses performance at the end of a half-year, a school year or of schooling. If a report or leaving certificate has been lost, a certificate of attendance does not help; an application for a replacement (Zweitschrift) does.

For absences through illness, a medical appointment that could not be moved or an urgent family matter. When and how an absence must be excused is set by each Land's school law: in North Rhine-Westphalia at once and in writing; in Baden-Württemberg the school must be told by the second day and, if that was not in writing, the written note follows within three days. If the school has closed, it goes to the school that keeps its records or to the school administration of its operator.
