The request to a Greek school for a certificate of studies, a certificate of attendance or a copy of the school-leaving certificate (apolytirio): the graduate's details as the school keeps them (father's and mother's names, date of birth, name while at school), the year of graduation, the document wanted, the number of copies and the purpose. Graduates from the 2016–2017 school year on can get their apolytirio from gov.gr; this request serves older titles and certificates that are not there.

Is a request to the school needed for a copy of a Greek school-leaving certificate obtained after 2016–2017?
Usually not. Graduates from the 2016–2017 school year on can issue a digital copy of their school-leaving certificate on gov.gr, with their TAXISnet credentials. A request to the school is needed for older titles, for a certificate of studies with the marks for each year, for a certificate of attendance and for anything gov.gr does not show.
What does someone do who has lost a Greek school-leaving certificate from an earlier year?
They send this request to the school they graduated from, asking for a copy of the title, which the school issues from its records. If the school also asks for a solemn declaration about the loss, it can be issued free on gov.gr. The details in the request — father's and mother's names, date of birth, year of graduation — help the school find the entry in its books.
Where does the request go if the Greek school has closed or merged?
To the school that took over its archive. If it is not known which one that is, the area's directorate of primary or secondary education can say where the archive is kept, and the request is filed there with the same details.
Can someone else file the request and collect the document on the graduate's behalf in Greece?
Yes, with an authorisation. The gov.gr authorisation is issued free with TAXISnet credentials, and every public body — the school included — must accept it. The authorised person shows their ID card when collecting.
