- Cohabitation agreement for unmarried partners (Poland)An agreement for couples living together in Poland without marrying: how they share housing and living costs, what each keeps as personal property, how things bought together are co-owned, how they settle up if they separate, and who leaves the home. Poland has no registered partnerships — the closest-person status law did not take effect after the Sejm failed to override the presidential veto in September 2026 — so these matters are settled by a civil-law contract.
- Demand to an employer for unpaid wagesAn employee's demand for unpaid wages in Poland: for which periods, how much, by when and to which account, reminding the employer that pay is due within the first 10 days of the following month, and saying that the labour court and the National Labour Inspectorate come next. It ends the silence and proves an attempt to settle amicably.
- Minutes of an annual shareholders' meeting with resolutions (Poland)Minutes of the annual shareholders' meeting of a Polish limited company with the three resolutions art. 231 of the Commercial Companies Code requires: approving the financial statements and the management report, distributing profit or covering a loss, and discharging the board members. It records the chair's election, the meeting's capacity to pass resolutions, the agenda and the votes. The meeting must be held within 6 months of the financial year's end.
- Application to change a first name or surname (Poland)An application to a Polish civil registry office (urząd stanu cywilnego) for an administrative change of first name or surname, with what the Act on changing names requires: the applicant's details and birth surname, the offices holding the birth and marriage certificates, the new name, any children it extends to, the important reason, and a statement that no application in the same matter was made elsewhere and no refusal was issued. It can be filed at any civil registry office.
- Flat handover record — move-inThe record made on the day a flat in Poland is handed to the tenant: the state of the flat, its furnishings, the electricity, gas, water and heating meter readings, the keys handed over and any defects noticed, signed by both sides. When the flat is returned its state is compared with this record — which is what decides which damage happened during the tenancy, and so what may be deducted from the deposit.
- Car sale agreement (umowa kupna-sprzedaży samochodu)A Polish agreement for the private sale of a used car: the parties, make and model, VIN, registration number, mileage, known defects, the price in figures and words, and handover with the documents and keys. It also sets out what each side must do afterwards: the seller tells the district office within 30 days and the insurer within 14, the buyer registers the car within 30 days and pays 2% tax on civil-law transactions within 14. No notary is needed.
- Special power of attorney for a specific act (pełnomocnictwo szczególne)A Polish power of attorney for an act beyond ordinary management — selling a car, signing a particular contract, collecting money, handling a specific matter — setting out the act, its limits, how long it lasts, and whether the agent may appoint a substitute or contract with themselves. Where the act needs a special form, the power of attorney needs the same one — selling real estate requires a notarial deed, as the document warns.
- Request to work remotely (wniosek o pracę zdalną)An employee's request in Poland to work remotely in full or in part: from when, where and on what pattern, with a statement about premises and equipment. Some employees — pregnant ones and parents of children up to 4, among others — are entitled to have the request granted unless the organisation or type of work rules it out; the letter lets you state that ground. Occasional remote work, up to 24 days a year, has its own request.
- Mutual non-disclosure agreement (umowa o zachowaniu poufności)A mutual NDA for two businesses in talks about cooperation, an investment or a deal: the purpose, a definition of confidential information linked to trade secrets under Polish law, the exceptions, rules on access and protection, return of materials, how long it lasts after the talks, and a contractual penalty with the right to claim damages above it. It creates no obligation to sign anything further.
- At-fault driver's statement after a collision (Poland)A statement written at the scene of a minor collision in Poland where nobody was hurt: the date, time and place, the at-fault driver's details and licence, their vehicle and OC policy, the other driver and vehicle, what happened, visible damage, witnesses, room for a sketch, and the admission of fault. It is the main evidence for a claim against the at-fault driver's insurer when the parties do not call the police.
- Consent for a child to travel abroad (zgoda na wyjazd dziecka za granicę)A parent's written consent for a minor to travel abroad from Poland — with the other parent, grandparents, a camp or anyone else: the parent's and child's details, the travel document, countries, dates and the adult in charge. Polish law sets no official form, but parents decide important matters for a child together, and border guards may check consent when a child travels without one parent. There is room below the signature for a notary's certification.
- Occasional tenancy agreement (najem okazjonalny)A Polish occasional tenancy for an owner who is a private individual not in the business of letting flats: fixed term of at most 10 years, deposit of up to six months' rent. The agreement lists the three compulsory attachments, including the tenant's declaration submitting to enforcement and undertaking to vacate the flat — which must be a notarial deed; without it there is no occasional tenancy. The landlord reports the agreement to the tax office within 14 days.
- Request to the local consumer ombudsman for help with a dispute (Poland)A request to the Polish municipal or county consumer ombudsman (rzecznik konsumentów), who helps free of charge with individual disputes with businesses — the ones UOKiK does not handle: the business's details, the facts and the correspondence so far, the outcome wanted and the kind of help to underline — advice, an approach to the business, or a claim brought for the consumer (art. 42 of the Competition and Consumer Protection Act).
- Application to annul a final fixed-penalty ticket (Poland)An application to the Polish district court to annul a fixed-penalty ticket (mandat) that has already been accepted — for a traffic offence, say — under art. 101 of the Code of Procedure for Petty Offences. It identifies the ticket, the date it became final, the statutory ground and the reasons. The grounds are narrow — an act that is not an offence, an act committed before the age of 17, a fine above the legal limit — and the deadline is 7 days from the ticket becoming final. The document says so plainly.
- Revocation of a power of attorney (odwołanie pełnomocnictwa)A principal's statement revoking a Polish power of attorney, saying when it was given and what it covered, demanding the document back (art. 102 of the Civil Code) and listing the people and institutions told of the revocation. It takes effect when it reaches the agent, but an act done with someone unaware of the revocation can still stand — so the bank, the office and business partners should be told too.
- Demand for return of a rental depositA tenant's letter to a landlord in Poland who has not returned the deposit after moving out: when the flat was vacated, how much is to be repaid, by when and to which account, with a request for a written account of every deduction. The landlord has a month from the flat being vacated; after that interest runs, and the letter also shows the attempt at an amicable solution a court will ask about.
- Notice terminating the current rent (rent increase)A rent increase for a flat in Poland done the way the law requires: not as a "notice of increase" but as written termination of the current amount of rent, with three months' notice (or longer under the agreement), to the end of a calendar month. The letter states the new rent and the date it applies from, and tells the tenant their rights — which is fair to them and makes the increase easier to defend.
- Demand for unpaid rent with warning of terminationThe letter without which a landlord in Poland cannot end a flat tenancy for arrears: it lists the unpaid periods and the amount, warns of the intention to terminate, and sets an additional deadline of at least a month to pay both arrears and current charges, as art. 11(2)(2) of the tenants' protection act requires. Notice given without this warning is defective.
- Vehicle handover record (protokół zdawczo-odbiorczy samochodu)A record of handing a car over to a buyer, hirer or other user in Poland, to go with the sale or rental agreement: the date and time of handover, the parties, the make, registration and VIN, the odometer and fuel level, the number of keys, the documents and equipment handed over, and visible damage. The handover time settles who answers for fines, charges and damage, and the description of condition heads off disputes about defects.
- Loan agreement (umowa pożyczki)A Polish agreement for a cash loan between private individuals, family included: the parties with their PESEL numbers, the amount in words, how and when it is paid out, the interest, the repayment date or schedule, late-payment interest and the account for repayment. It explains the tax on civil-law transactions: 0.5% and a return within 14 days, with close family exempt up to 36,120 zł without formalities, and above that only after filing and a bank transfer.
- Notice terminating a telecoms contract (Poland)Notice ending a contract with a Polish phone, internet or TV operator: the customer and contract numbers, the service and number, the basis for ending it to underline — a contract rolled over after the promotional term, an open-ended one, early termination, or a change of terms by the operator — the date wanted, and the return of equipment and any overpayment. It states the one-month notice for a rolled-over contract under the Electronic Communications Law.
- Pre-action demand for payment between businesses (Poland)A creditor business's demand to a trading partner for overdue invoices: the principal, the list of invoices, statutory late-payment interest for commercial transactions and the fixed compensation for recovery costs — the equivalent of 40, 70 or 100 euro per payment — the deadline and the account. It is the attempt at amicable settlement the court asks about when a claim is filed (art. 187 § 1(3) of the Code of Civil Procedure).
- Claim against the at-fault driver's motor insurer (Poland)The injured party's claim to the at-fault driver's insurer: the date and place, the at-fault driver, their vehicle and policy number, the injured party's vehicle, what happened, the damage, the evidence, the amount claimed and the account for payment, reserving the right to a replacement-car hire refund. It reminds the insurer that compensation is paid within 30 days of the claim, and filing the claim interrupts the limitation period.
- Request to correct a certificate of employment (świadectwo pracy)A request to the employer to correct errors in the Polish certificate of employment — the wrong period of employment, the wrong way the contract ended, the wrong amount of leave taken. It is made within 14 days of receiving the certificate; if the employer refuses, a claim can go to the labour court within 14 days of being told. The letter says what is wrong, what it should say, and why.
