This document is in Italian

The PDF you download is in Italian — that is the language it has to be filed in, so it is not translated. Everything on this page is here to tell you what it says.

An Italian declaration, in Italian (art. 47 of Presidential Decree 445/2000), attesting facts, status and qualities you know directly that are not on the self-certification list, including about other people: for example the loss of a document, to ask for a duplicate, or a factual situation a call for applications or a body asks for. With the warning on penalties and the note on signing. Many public bodies publish free models. No stamp duty is due (art. 37).

Common questions

Can the sworn statement be used for a lost document in Italy?
Yes, to ask for a duplicate of an identity document or one attesting personal status and qualities, unless the law expressly requires a report to the police (art. 47(4) of Presidential Decree 445/2000). For the identity card and the passport, the offices ask for the police report: since 15 June 2026 the State Police takes it free online on its Denunce online portal.
When must the signature on the sworn statement be authenticated?
Towards a public body it is enough to sign it in front of the official or attach a copy of an identity document (art. 38). If, however, the statement is given to a private party, or lets a third party collect money, such as an heir at a bank, the signature must be authenticated by a notary, a court clerk, the municipal secretary or an official the mayor appoints (art. 21(2)).
What should a sworn statement of personal knowledge contain?
Only facts you know directly, stated precisely: dates, places, people, documents. If they concern other people, say how you know them. Do not put health data in it, as medical certificates cannot be replaced by a declaration (art. 49). A false statement is a criminal offence and loses any benefit obtained (arts. 75 and 76).

Many banks also have their own form.

Banks and Poste Italiane may ask for their own forms. Signed by the principal and accepted by the agent.

The body answers within 30 days. Many bodies publish a free form. No revenue stamp is needed to view documents or for plain copies; certified copies need one (16 €, from a tobacconist or with @e.bollo), and there is space to affix it. Send it by PEC or hand it in at the office.

  • Application to the Prefetto to change a first name or surname (Italy)

For an adult Italian citizen asking to change their surname or first name, or to add one, such as the mother’s surname. The application goes to the Prefetto of the province of residence or of the one holding the birth record (article 89 of Italian Presidential Decree no. 396/2000), with the reasons and the name sought. Many Prefetture publish their own free form: if there is one, you may use it. It needs a € 16 revenue stamp (marca da bollo, from a tobacconist; online via @e.bollo or F24), except for ridiculous or shameful surnames.

The body answers within 30 days; a refusal can be reviewed by its transparency officer. Release is free apart from the cost of copies on paper or media: no revenue stamp. Many bodies have a free form in their ‘Amministrazione trasparente’ section. Send it by PEC or e-mail.
