The application by which a parent excuses a child's absence for family reasons. How many days a parent may excuse themselves, and from which point the head teacher's permission is needed, is set by each school's own rules — check yours before filling in the dates. It is filed in advance, not after the child is back in class.

- How many days can a child be absent for family reasons?
- The rules give the class teacher power to allow a small number of days and the head teacher more. The exact figures are in each school's own rules, which is why the form leaves them as a field rather than assuming them.
- Do I have to file the application in advance?
- Yes, where the absence is planned. Filed afterwards, it turns what would have been a permitted absence into an unexcused one, however good the reason.
- Does it matter how many unexcused absences build up?
- Yes. Unexcused absences lead to sanctions under the school rules, and past a certain number to family benefits being stopped. So it is worth putting the absence through the proper route rather than explaining it afterwards.
- Is it addressed to the class teacher or the head?
- It depends on the number of days. For a few days the class teacher is enough; beyond that the head decides. Filing with the wrong one is the usual reason an application comes back without being considered.
