For people in Switzerland moving to another health insurer: the letter ending the compulsory basic insurance either after the new premium has been announced, for the end of the year (Art. 7 para. 2 LAMal/KVG), or with three months' notice for the end of June or December, for yourself and children covered with you. It names the new insurer and leaves supplementary cover in place. To change on 1 January, the insurer must receive it by 30 November — the postmark is irrelevant. The Federal Office of Public Health has a free model letter on priminfo.admin.ch. Send it registered or by A-Post Plus. In Italian.

Common questions

Does the Swiss government offer a free letter for cancelling basic health insurance?
It does. The Federal Office of Public Health puts free model letters on priminfo.admin.ch under «Modelli di lettere» in Italian — one cancelling the basic insurance, one applying to the new insurer — next to its premium calculator for comparing offers. Many insurers have their own forms as well. This version is a fill-in letter with room for children covered with you, the new insurer and any supplementary cover.
What is the deadline for a Swiss health insurer to receive notice for 2027?
To move on 1 January 2027 after the premium announcement, your insurer must have the notice by Monday 30 November 2026; receipt is what counts, not the postmark (Art. 7 para. 2 LAMal/KVG). The Federal Office of Public Health advises posting it registered or by A-Post Plus by 15 November. With the standard deductible and free choice of doctor you can also leave at the end of June, the notice arriving by 31 March (Art. 7 para. 1). With a higher deductible or a restricted-choice model (from 2027 called PRAXIS, TEL_DIG, PHARM or FLEX, such as family-doctor, HMO or telemedicine models), only at the end of the year (Art. 94 para. 2 and Art. 100 para. 3 OAMal/KVV).
Which unpaid bills can stop a change of Swiss health insurer?
For a change on 1 January: premiums, cost-sharing or default interest for which you were reminded by 30 November, and enforcement costs, if they are still unpaid on 31 December (Art. 64a para. 6 LAMal/KVG, Art. 105l para. 2 OAMal/KVV); a child's arrears block only that child's change. Apply to the new insurer at the same time as you give notice: in basic insurance it must take you without reservations or health questions. Your old cover ends only once the new insurer has told the old one that you are insured without a gap; the old insurer then tells you the end date in writing (Art. 7 para. 5 LAMal/KVG).
Will my supplementary cover end if I leave my Swiss basic insurer?
No. Your insurer cannot make you drop supplementary insurance when you leave its basic insurance, and cannot cancel it itself for that reason alone (Art. 7 paras. 7 and 8 LAMal/KVG). Supplementary policies follow their own notice periods under the Insurance Contract Act and the general conditions. Keep them until new cover is confirmed, since a supplementary insurer may ask about your health and refuse you.
