To change on 1 January, the notice must reach the insurer by 30 November; the postmark does not count. The Federal Office of Public Health has a free model letter on priminfo.admin.ch. Common questions

Does the Federal Office of Public Health offer a free French model letter for leaving a basic insurer?
Yes. The Federal Office of Public Health (OFSP) publishes free model letters on priminfo.admin.ch under « Lettres types » — among them one terminating the compulsory basic insurance — next to the premium calculator for comparing insurers. Many insurers also have their own forms. This letter is a fill-in version with fields for children insured with you, the new insurer and your supplementary cover.
When exactly must a Swiss basic insurer receive my notice to change on 1 January 2027?
By Monday 30 November 2026 — receipt counts, not the postmark (Art. 7 para. 2 LAMal). The Federal Office of Public Health advises posting it by 15 November, registered or by A-Post Plus. With the ordinary deductible and free choice of doctor you can also change for the end of June, the notice arriving by 31 March (Art. 7 para. 1 LAMal). With a higher optional deductible or a restricted-choice model (from 2027 called PRAXIS, TEL_DIG, PHARM or FLEX, such as family-doctor, HMO or telemedicine models), only for the end of the year (Art. 94 para. 2 and Art. 100 para. 3 OAMal).
What can block a change of basic insurer in Switzerland, and when does the old cover stop?
Join the new insurer at the same time; in basic insurance it must accept you without reservation and without health questions. The old insurance ends only once the new insurer has told the old one that cover continues without a gap; the old insurer then tells you the end date (Art. 7 para. 5 LAMal). A change on 1 January is blocked by premiums, cost-sharing or default interest reminded by 30 November, and by enforcement costs, if still unpaid on 31 December (Art. 64a para. 6 LAMal, Art. 105l para. 2 OAMal). A child's arrears block only that child's change.
Should I cancel supplementary cover when I leave my basic insurer in Switzerland?
No. The insurer cannot make you cancel supplementary insurance, nor cancel it itself merely because you changed basic insurer (Art. 7 paras. 7 and 8 LAMal). Supplementary insurance has its own notice periods under the Insurance Contract Act (LCA) and the policy conditions. Cancel it only once new cover is certain: a supplementary insurer may ask health questions and refuse an application.
