The PDF you download is in Italian — that is the language it has to be filed in, so it is not translated. Everything on this page is here to tell you what it says.

For a private-sector employee in Italy whom INPS has granted the paid leave of art. 33 of Italian Law 104/1992, to care for a relative with a severe disability or for themselves. The application goes first to INPS, online and free, or through a patronato; this letter then gives the employer the details of the authorisation, with a copy enclosed, and the monthly schedule of days or hours, which the employer may ask for. The letter does not describe the disability. Send it by registered post, PEC or by hand. In Italian.

Common questions

How is leave under Italy's Law 104 obtained?
A private-sector employee applies to INPS online, with SPID, CIE or CNS, or free of charge through a patronato; the medical board's report recognising the severe disability is needed. Once authorised, the employee gives the employer the details of the decision, as this letter does. A public-sector employee applies instead to their own administration, as it directs.
Can an employer in Italy ask for Law 104 leave to be scheduled?
Yes. According to Italy's Ministry of Labour (ruling no. 31/2010) the employer may ask for a schedule, for example monthly, if the worker can name the days of absence in advance and the request does not compromise the care given. The criteria should be agreed with the workers or their representatives as far as possible, and the collective agreement may regulate them. A sudden need for care allows the day to be changed, with notice as soon as possible.
How much leave does Italy's Law 104 give, and is it paid?
A worker caring for a relative with a severe disability has three days of paid leave a month, which may also be taken by the hour (art. 33(3) of Italian Law 104/1992); a worker with a severe disability may choose between two hours a day and three days a month (art. 33(6)). The leave is paid and credited for pension purposes. This letter can be sent again each month with the new schedule.

It is for cases where the collective agreement or the company asks for written notice and offers no portal of its own.
