Application for a duplicate school certificate (Poland)
An application to a Polish head teacher for a duplicate of a lost or damaged certificate — a school-leaving, end-of-year or matura certificate: the type and the year issued, the surname at the time, date of birth, class or programme, the reason, proof of the 26 zł fee, and how it will be collected. A duplicate has the force of the original, and if the school no longer exists, the body holding its records issues it.
Request for access to medical records (Poland)
A request to a Polish hospital, clinic or practice for a patient's medical records: the patient's details and PESEL, what records and which treatment period, who is asking — the patient, a legal representative, an authorised person or a close relative of a deceased patient — and the form to underline: inspection, a paper or electronic copy, an extract or a transcript. It reminds the provider that the first copy in the requested scope is free for the patient (Patient Rights Act).
Request to the local consumer ombudsman for help with a dispute (Poland)
A request to the Polish municipal or county consumer ombudsman (rzecznik konsumentów), who helps free of charge with individual disputes with businesses — the ones UOKiK does not handle: the business's details, the facts and the correspondence so far, the outcome wanted and the kind of help to underline — advice, an approach to the business, or a claim brought for the consumer (art. 42 of the Competition and Consumer Protection Act).
Freedom of information request (wniosek o udostępnienie informacji publicznej)
A request under Poland's Access to Public Information Act of 6 September 2001, to an office, municipality, school, municipal company or other body performing public tasks: the information sought, the form and way it should be provided, and a reminder of the 14-day deadline and that no legal or factual interest need be shown. No answer in time opens the way to a complaint about inaction to the regional administrative court.
Application to pay tax arrears in instalments (Poland)
An application to a Polish tax office or other tax authority to spread tax arrears and interest over instalments (art. 67a § 1(2) of the Tax Ordinance): the tax and period, the amount, the proposed number of instalments and schedule, and above all the taxpayer's important interest that justifies it, with the household's income and outgoings. The relief is discretionary, so a thorough justification decides the outcome.
Application to change a first name or surname (Poland)
An application to a Polish civil registry office (urząd stanu cywilnego) for an administrative change of first name or surname, with what the Act on changing names requires: the applicant's details and birth surname, the offices holding the birth and marriage certificates, the new name, any children it extends to, the important reason, and a statement that no application in the same matter was made elsewhere and no refusal was issued. It can be filed at any civil registry office.
Request to work remotely (wniosek o pracę zdalną)
An employee's request in Poland to work remotely in full or in part: from when, where and on what pattern, with a statement about premises and equipment. Some employees — pregnant ones and parents of children up to 4, among others — are entitled to have the request granted unless the organisation or type of work rules it out; the letter lets you state that ground. Occasional remote work, up to 24 days a year, has its own request.
Parental leave request (wniosek o urlop rodzicielski)
An employee's request for parental leave in Poland: the child and date of birth, the length in weeks, the dates, what the other parent has taken, and the attachments. Parental leave is 41 weeks for both parents together (43 for more than one child), 9 weeks of which are reserved for each parent. The request goes in at least 21 days before the leave starts.
Request to an employer for a reference letter
A courteous, practical request to a current or former employer in Poland for a reference: the position and period worked, what it is needed for and what is worth highlighting. In Poland a reference is voluntary — only the certificate of employment is compulsory — so the letter makes agreeing easy by suggesting specific achievements and a date.
Request to correct a certificate of employment (świadectwo pracy)
A request to the employer to correct errors in the Polish certificate of employment — the wrong period of employment, the wrong way the contract ended, the wrong amount of leave taken. It is made within 14 days of receiving the certificate; if the employer refuses, a claim can go to the labour court within 14 days of being told. The letter says what is wrong, what it should say, and why.
Unpaid leave request (wniosek o urlop bezpłatny)
An employee's written request for unpaid leave in Poland: dates, number of days and reason. The employer may grant it but does not have to; there is space for the decision on the same sheet. Unpaid leave does not count towards the service on which employee entitlements depend, and for leave over three months the parties can agree that the employee may be recalled earlier for important reasons.
Annual leave request (wniosek o urlop wypoczynkowy)
An employee's request for annual leave in Poland: for which year, how many working days and when, with space for the employer's decision on the same sheet. Leave is 20 days with under 10 years' service and 26 days with at least 10 years, and time in education counts towards that. Leave on demand — up to 4 days a year — is requested on the day it starts at the latest.
Request for the landlord's consent to sublet a flat
A tenant's request in Poland for written consent to sublet the flat or part of it: to whom, for how long, for what rent and why, with confirmation that the tenant's obligations to the landlord stay with the tenant. There is room at the foot for the landlord's signature — a signed copy is the proof of consent, because subletting without it is one of the grounds on which the landlord may end the tenancy with a month's notice.
