The PDF you download is in Polish — that is the language it has to be filed in, so it is not translated. Everything on this page is here to tell you what it says.

An employee's request in Poland to work remotely in full or in part: from when, where and on what pattern, with a statement about premises and equipment. Some employees — pregnant ones and parents of children up to 4, among others — are entitled to have the request granted unless the organisation or type of work rules it out; the letter lets you state that ground. Occasional remote work, up to 24 days a year, has its own request.

Common questions

Who must be allowed to work remotely in Poland?
The employer grants the request of, among others, a pregnant employee, an employee raising a child up to age 4, a parent of a disabled child and an employee caring for a close family member with a disability certificate (art. 67¹⁹ § 6 of the Labour Code). It may refuse only if remote work is impossible because of the organisation or type of work, giving the reason within 7 working days.
How does occasional remote work differ from regular remote work?
Occasional remote work is done at the employee's request, on paper or electronically, for up to 24 days in a calendar year (art. 67³³ of the Labour Code), with no standing arrangement. The employer does not have to cover costs such as electricity or internet for it, while for regular remote work it does.
Who pays for electricity and internet when working remotely?
The employer. It covers the necessary costs of remote work — electricity, telecoms, materials and tools — or pays an agreed flat amount matching the expected costs (art. 67²⁴ of the Labour Code). The rules are set by agreement with employee representatives, workplace rules, or an instruction or agreement with the employee.
