The PDF you download is in Polish — that is the language it has to be filed in, so it is not translated. Everything on this page is here to tell you what it says.

A record of taking in a phone, computer, bike or appliance for repair, issued to the customer by the workshop or drawn up by the customer for the workshop to sign: the job number, the workshop's and customer's details, the item with its serial number, accessories, visible condition, the fault reported, the estimated cost and time, and a rule that more expensive work needs the customer's consent. At collection it proves the condition the item was left in.

Common questions

Can a Polish repair shop charge more than its estimate?
With a lump-sum price, as a rule not, even if the work proved more costly (art. 632 of the Civil Code). With a cost estimate the shop drew up itself, it can ask for more only if, despite due care, it could not have foreseen the extra work, and never for extra work done without the customer's consent (art. 630). If costs rise significantly, the customer can withdraw, paying a fair part of the price (art. 631). That is why it is worth writing into the receipt that more expensive work needs the customer's consent.
What if the fault returns after a repair in Poland?
Complain to the workshop about the repair, describing what does not work and asking for a free repeat repair. A consumer whose complaint the workshop does not answer within 14 days can treat it as accepted (art. 7a of the Consumer Rights Act). If the repair was under guarantee, the complaint goes to the guarantor, and in a dispute the local consumer ombudsman can help.
Why describe the item's appearance and accessories when leaving it for repair?
Because at collection it is the only proof of the condition it was left in. Noting scratches, cracks and dents and listing accessories — a charger, case, memory card — makes it possible to show that new damage or a missing item arose at the workshop, which is responsible for what it was entrusted with. It is also worth taking photos at drop-off and backing up data beforehand, since the repair may require erasing it.
