The record describes the item, the visible damage at the point of handover and the fault reported — and fixes whether a deadline and an estimated price were given. Common questions

The repair shop only gave me a numbered slip. Is that enough?

It proves the item was handed in, but not the condition it was in. If there is a new scratch or a missing part on return, the argument is conducted with no description — which is exactly what this record supplies.

Why describe the damage the item already has?

So that it is clear which damage was pre-existing. What is described at handover protects both sides: you from the claim that it was always like that, and the shop from a claim for something it did not cause.

Can the price change during the repair?

It can, if something further is found — which is why there is a field for an estimate and for approving costs above it. Without such a term the final bill becomes a matter of dispute while the item is in someone else's hands.

Is there a warranty on the repair itself?

Usually yes, on the work done and the parts fitted, and it is separate from any warranty on the item. Write the period in — without it a later complaint about the same fault starts from scratch.
