Rent paid in cash without a receipt cannot be proved. The receipt certifies which period was paid for, how much of it was rent and how much utilities, and that the obligations for that period are discharged. It is issued by the landlord on taking the money and kept by the tenant.

Is a receipt needed if I pay by bank transfer?

With a bank transfer the statement does the same job, provided the reference is filled in. A receipt is effectively essential for cash, where otherwise nothing at all records that the money changed hands.

What does the receipt prove if the landlord says I have not paid?

That for the stated period the obligation is discharged, and by how much. That is why it separates the rent from the utilities — in a dispute the usual assertion is that what was paid covered only one of them.

Can one receipt cover several months?

It can, if the period is written out clearly. It is safer to have a separate receipt for each payment — that way an argument about one month does not reopen the whole year.

Who signs it?

The landlord, because they are the one certifying that they received the money. The space for the tenant's signature confirms the money was handed over — but the evidential weight is carried by the recipient's signature.
