Find the document you need
Answer three short questions. You don't need to know what the form is called.
1. Which country is it for?
2. What is it about?
3. What do you want to do?
15 documents match
- Declaration of family and financial circumstancesAsked for when applying for social assistance, legal aid, a grant, a reduced fee or municipal housing. Many institutions have a form of their own — if there is one, use it; this document is for when there is not, or when you want to prepare the figures in advance. It is declared under liability, so set out your income in full.
- Receipt for documents handed overFor every time you leave originals with someone else — an accountant, a lawyer, an employer, a bank, an agent. The receipt lists the documents one by one, marks which are originals and which copies, and gives a date for their return. An original handed over without a receipt is later chased with no evidence that it was ever handed over at all.
- Vehicle handover recordThe record made when a car changes hands — on a sale, at the start of a rental, or on collection after a repair. The odometer reading and the description of the visible damage at that moment are the only things that later tell existing damage apart from damage caused since. The record also lists what was handed over: keys, registration document, insurance policies, tyres, jack, first-aid kit.
- Declaration under Art. 141(8) of the Commerce ActThe declaration every new managing director files — that they meet the requirements of the law and that nothing bars them from holding the post. It is filed when the company is incorporated and on every later change of managing director.
- Receipt for money receivedWritten evidence that a sum has been paid and received — on a private sale, a deposit, rent or a loan. It is completed by the person receiving the money and signed as the money changes hands.
- Minutes of a general meeting of the owners' associationDecisions of a general meeting can only be proved by the minutes, and minutes with no quorum, no announced agenda or no recorded votes are successfully challenged. So each of those is a separate field here. The Condominium Management Act sets requirements for both the calling of the meeting and the content of the minutes — check them before the meeting, because afterwards they cannot be put right.
- Receipt for a discharged debtThe document a debtor should ask for at the moment of the final payment, and then keep. The receipt identifies which obligation has been discharged, with what sum, and that the creditor has no further claim under it. Without one, a debt that has been paid goes on existing on paper — including if the creditor assigns it to someone else.
- Resolution of the sole owner of the capitalThe resolution by which the owner incorporates the company, adopts the constitutive act, fixes the registered seat and the capital, and appoints a managing director. It is filed together with the constitutive act and repeats the same particulars — fill both in at one sitting so that they cannot disagree.
- Receipt for a returned depositThe document that closes a tenancy. The receipt shows the deposit that was paid, the sums withheld and the ground for each, and states expressly whether any claims remain. If a landlord withholds part of a deposit without setting that out, the argument afterwards is conducted with no record from either side.
- Rent receiptRent paid in cash without a receipt cannot be proved. The receipt certifies which period was paid for, how much of it was rent and how much utilities, and that the obligations for that period are discharged. It is issued by the landlord on taking the money and kept by the tenant.
- Declaration under Art. 13(4) of the Commercial Register ActA mandatory attachment to every filing with the Commercial Register: that the circumstances declared exist and that the documents attached are genuine. It is short, but a filing does not go through without it — and nowhere is it explained who exactly signs it. The applicant does, meaning the person making the filing.
- Property handover recordThe document that decides whose the deposit is. It records the readings of every meter, the number of keys handed over, the contents and the condition of the property at that moment. It is completed both on moving in and on moving out — comparing the two records is the only objective evidence of what changed.
- Record of an item handed in for repairYou are leaving a phone, a laptop, an appliance or a machine at a repair shop. The record describes the item, the visible damage at the point of handover and the fault reported — and fixes whether a deadline and an estimated price were given. It is also the document a later complaint about a poor repair rests on, because it shows the condition the item was in when it went in.
- Declaration under Art. 313 of the Criminal CodeThe all-purpose declaration that institutions and employers ask for most often. You fill in your details and the wording of what you are declaring; the clause on criminal liability for a false declaration is already in the text.
- Record of acceptance of completed workThe other half of a contract for work. The record fixes what has been accepted, what has been noted as defective, what remains to be put right and how much is owed. Without it, an argument about whether the work was finished is conducted from memory. It is signed by both sides at the moment of acceptance, not afterwards.
