Request for a pension statement, contribution record or account review (Germany)
The letter to the German statutory pension insurance (Deutsche Rentenversicherung) asking for a pension statement (Rentenauskunft), an up-to-date contribution record (Versicherungsverlauf) or a review of your insurance account (Kontenklärung) — with your insurance number, birth name and date of birth, the reason and the periods you believe are missing, such as school, university, child-raising or work abroad. The pension insurance also offers these requests free online; this letter is for those who prefer to apply in writing, cannot use the online services or live abroad. Application for exam adjustments at university for a disability or chronic illness (Germany)
The application to a German university's examination board for exam adjustments (Nachteilsausgleich) for a student with a disability or chronic illness: more time, breaks, a separate room, technical aids or another form of exam, without lowering the standard. Exam regulations must take the needs of disabled students into account (§ 16, fourth sentence, of the German Higher Education Framework Act, HRG). It gives the student number, the programme, the exams concerned, how the impairment affects them and the adjustments requested, with a medical certificate enclosed. Application to the energy conciliation body, Schlichtungsstelle Energie (Germany)
An application for conciliation to the Schlichtungsstelle Energie e.V. in Berlin, for when your electricity or gas supplier, network operator or meter operator in Germany rejected your complaint or did not answer within four weeks (§§ 111a and 111b of the German Energy Industry Act, EnWG). The procedure is free for consumers and the company must take part. The letter sets out the dispute, your earlier complaint, the company's answer and what you want, and states that no court is involved. Application for adjustments at school for a pupil's disability or impairment (Germany)
The application to a school's head in Germany for adjustments (Nachteilsausgleich) for a pupil with a disability, a chronic illness or an impairment such as dyslexia: adapted conditions for tests and exams — more time, breaks, a separate room or aids — without lowering the standard expected. It describes how the impairment affects the pupil's work, the adjustments requested and the period, and encloses a medical or specialist report. Request to inspect and copy the medical record (Germany)
The letter to a doctor's practice, hospital or therapist in Germany asking to inspect your treatment record (Behandlungsakte, formerly Patientenakte) and for a complete copy. It relies on § 630g of the German Civil Code (BGB) as in force since 6 February 2026 — the first copy is free, electronic ones included — and on Art. 15 GDPR with its one-month deadline. Freedom of information request (Germany)
The request for access to official information held by a German authority — files, contracts, reports, e-mails or answers to questions: under the Federal Freedom of Information Act (IFG) from federal bodies, under the Land's freedom-of-information or transparency act from Land and municipal bodies, or under the Environmental Information Act (UIG) or the Consumer Information Act (VIG). You choose the act and the form of access; the letter asks for an answer within the statutory time limit, for notice of any fees in advance and for partial access rather than refusal. In German.
Pre-approval request and claim to a private health insurer (Germany)
The letter to a German private health insurer (PKV) in two situations: before expensive treatment, as a pre-approval request under § 192(8) of the Insurance Contract Act (VVG) — where costs are expected to exceed €2,000 the insurer must answer with reasons within two weeks if the treatment is urgent, otherwise four, or the treatment is presumed necessary — or afterwards, as the covering letter to a claim with the invoices. With policy number, treatment, costs and IBAN; no diagnosis in the letter. In German.
Request for a certificate of school attendance (Germany)
The letter to a school office in Germany by which parents, or pupils who are of age, ask for a certificate of attendance (Schulbescheinigung) — the confirmation that a child attends the school, as needed for example for child benefit for a child over 18, for education grants or for a pupil's travel pass. It gives the pupil's name, date of birth, class and school year, the office the certificate is for, the number of copies and the date it is needed by, and asks the school to complete that office's own form if one is enclosed. Application for an apostille (Germany)
The letter asking the competent German authority to issue an apostille under the Hague Convention of 5 October 1961 for a German public document — a birth or marriage certificate, a notarial deed or a certificate of good conduct — so that it is recognised abroad. It identifies the document, the body that issued it and the country where it will be used, and says how it should be returned. Which authority is competent depends on the document; within the EU, Regulation (EU) 2016/1191 removes the apostille for many documents. Data subject access request under Article 15 GDPR (Germany)
Your request to a company, club or other body in Germany that processes data about you: under Article 15 of the GDPR (DSGVO in German) you ask what data it holds, why, who receives it, how long it keeps it and where it came from, and for a copy of the data — the first copy free. The answer is due within one month (Article 12(3)). If none comes, you can complain to the data protection authority of your German state. For customer, member and user accounts. In German.
Invitation letter for a visitor visa (Germany)
The letter in which you invite a relative or friend from a country whose nationals need a visa to visit you in Germany, addressed to the German embassy or consulate-general: details of the person invited and of you, the purpose and dates of the visit, where they will stay and who bears the costs. No form is prescribed for it. It is not a formal guarantee of costs (Verpflichtungserklärung) under § 68 of the German Residence Act (AufenthG): that is made separately at the foreigners' authority, on its official form, if the mission asks for one. Request to convert an account into a garnishment-protected P-Konto (Germany)
A letter asking your German bank or Sparkasse to run your current account as a garnishment-protected account (Pfändungsschutzkonto, P-Konto) — possible at any time, even when the account is overdrawn or already garnished (§ 850k of the German Code of Civil Procedure, ZPO). The bank must switch at the latest from the fourth business day after the request, and protection applies retroactively if you ask within a month of the garnishment. It includes the assurance that you hold no other P-Konto and covers the certificate of higher amounts (§ 903 ZPO). Since 1 July 2026, €1,590 a month is protected; the figure changes every 1 July. Conciliation application to the federal Universal Conciliation Body, with a consumer-centre complaint (Germany)
The Universalschlichtungsstelle des Bundes in Kehl, Germany's catch-all consumer conciliation body, offers its own free online form. This document is your written application for it: the facts, your claim and its value, your earlier attempt with the trader and the declarations needed, to upload, email or post. The procedure is free for consumers, covers claims from €10 to €50,000 where no sector body is competent, and needs the trader to take part. Request for a pupil's leave of absence from lessons (Germany)
The written request to a school's head in Germany to grant a pupil leave from lessons for an important reason, such as a family event that cannot be moved. It gives the name, class, dates, reason and evidence, states that the leave is not for extending the holidays, and asks for a written decision before it starts. Each Land's school law decides who grants leave and how early to apply; you choose the Land on the form. Missing school without permission to extend the holidays can lead to a fine. Application for a care level (Germany)
The application to a German long-term care insurer (Pflegekasse) for care-insurance benefits and the assessment that sets a care level (Pflegegrad) — for yourself or, with a power of attorney or a carer's card from the court, for someone else. An informal application is enough, but only a written one proves the date from which benefits run (§ 33 of Book XI of the Social Code, SGB XI). It flags the shorter deadlines after a hospital stay or announced care leave, and adds a ready letter claiming €70 per week begun if the insurer misses its 25 working days. In German.
Application to the health insurer to cover or refund costs (Germany)
The application asking a German statutory health insurer (Krankenkasse) to pay for treatment, a medical aid or another service — before treatment, as a request for cost cover with the three-, five- or six-week decision deadline of § 13(3a) of Book V of the Social Code (SGB V), or afterwards, as a claim for reimbursement where the insurer missed the deadline, could not provide urgent care in time or wrongly refused it. Request for a replacement school certificate (Germany)
The letter asking your former school in Germany for a replacement (Zweitschrift) of a lost or damaged certificate, such as the Abitur certificate or a school-leaving certificate. It names the certificate, the year it was issued, the years at the school and your name at the time, asks for your present name after a change of name, and asks about any fee beforehand. If the school has closed, it goes to the school that keeps its records or to the school administration of its operator. Claim to a childcare place: nursery or all-day primary care (Germany)
The letter to the local youth welfare office (Jugendamt) by which parents in Germany claim their child's right to a childcare place under § 24 of Book VIII of the German Social Code (SGB VIII): a nursery place or a childminder from the first birthday and — new since 1 August 2026 — all-day care for children who start Year 1 from the 2026/2027 school year. It gives the start date, the hours needed and the places already tried, and asks for confirmation of receipt and a written decision. The dated letter is the proof that counts if no place is found. Request to reduce income tax advance payments (Germany)
Your request to the German tax office (Finanzamt) to bring the quarterly income tax advance payments into line with lower income under § 37(3) of the German Income Tax Act (EStG) — after fewer orders, higher business costs, parental leave, illness or the end of an activity. The letter gives the year, the current and the requested payment and the expected income, explains the change, covers the solidarity surcharge and church tax, and asks for overpaid amounts to be refunded. Application for a tax clearance certificate (Germany)
Your application to the German tax office (Finanzamt) for a certificate on your tax affairs (Bescheinigung in Steuersachen), often called a tax clearance certificate — asked for in public tenders, for a restaurant licence and other trade permits, and by banks. It shows whether any tax is in arrears and whether returns have been filed, as at the day it is issued. The letter gives the tax number, the purpose and the date by which you need the certificate. Request to reduce working hours: part-time or bridging part-time (Germany)
An employee's request in Germany to cut working hours, permanently under § 8 of the Part-Time and Fixed-Term Employment Act (TzBfG) or for one to five years as bridging part-time under § 9a TzBfG, with current and desired weekly hours, the start date, the end date for bridging part-time and the preferred spread over the week. It reaches the employer in text form at least three months before the start; unless the employer refuses in text form a month before, the hours are reduced as asked. Requires over six months' service and over 15 employees (over 45 for bridging part-time). In German.
Unpaid leave request (Germany)
A request to a German employer for unpaid special leave (unbezahlter Sonderurlaub) over set dates, giving the reason and the terms both sides agree: release from work without pay, the employment continuing, and a return on the same terms. There is no general statutory right to it, so the leave exists only once the employer agrees, and the document has room below for that consent. It points out that social insurance cover continues for one month at most (§ 7(3) of the German Social Code, Book IV, SGB IV), after which health insurance must be arranged. In German.
Annual leave request (Germany)
A request to a German employer for annual leave (Erholungsurlaub) over set dates, with the number of working days and who covers the work, citing § 7(1) of the Federal Leave Act (BUrlG): the employee's wishes must be followed unless urgent operational needs, or colleagues with a socially stronger claim, stand in the way. Application to change a surname or first name (Germany)
A written application to the German name-change authority to change a surname, a first name or both under the Name Change Act (NamÄndG): the applicant's details, the name wanted, the important reason § 3 NamÄndG requires, the children the change extends to under § 4, and the enclosures. Not for returning to a birth name after divorce, double names (possible since 1 May 2025) or a change under the Self-Determination Act (SBGG) — those are declarations at the register office (Standesamt).
