Find the document you need
Answer three short questions. You don't need to know what the form is called.
1. Which country is it for?
2. What is it about?
3. What do you want to do?

- Declaration of family and financial circumstancesAsked for when applying for social assistance, legal aid, a grant, a reduced fee or municipal housing. Many institutions have a form of their own — if there is one, use it; this document is for when there is not, or when you want to prepare the figures in advance. It is declared under liability, so set out your income in full.
- Receipt for documents handed overFor every time you leave originals with someone else — an accountant, a lawyer, an employer, a bank, an agent. The receipt lists the documents one by one, marks which are originals and which copies, and gives a date for their return. An original handed over without a receipt is later chased with no evidence that it was ever handed over at all.
- Contract of mandateFor when you instruct someone to act on your behalf — to represent you before an institution, to manage a property, to negotiate, to receive payments. It differs from a contract for work and from a service agreement in that the agent acts for your account and owes you an account of it. The contract settles the scope, the reporting and the expenses; for acting towards third parties a separate power of attorney is also needed.
- Report to the manager of the owners' associationFor leaks in the common parts, broken lighting, a lift out of service, mess, or persistent breaches of the house rules. The difference between a grumble and a report is that a report is dated and describes the problem specifically — since when, where exactly, and what damage it is causing. The manager's duty to act starts running from its receipt, not from the first complaint on the stairs.
- Request for erasure of personal dataThe right to be forgotten under Article 17 of the General Data Protection Regulation. It is not unconditional: data the organisation keeps by law, for accounting, or to defend a legal claim stay. So the request names exactly which data and on what ground, and asks for a written answer — including when the answer is a refusal, so that there is something to appeal against.
- Objection to the insurer about the amount of a settlementFor when the settlement offered falls short of what the repair actually costs. The objection identifies the claim, the figure you are objecting to, and the ground — a garage quotation, an independent valuation, items left out of the schedule. It also asks for a copy of the expert assessment the insurer relied on, since without it there is no way to contest a particular item.
- Vehicle handover recordThe record made when a car changes hands — on a sale, at the start of a rental, or on collection after a repair. The odometer reading and the description of the visible damage at that moment are the only things that later tell existing damage apart from damage caused since. The record also lists what was handed over: keys, registration document, insurance policies, tyres, jack, first-aid kit.
- Notice of resignation (Art. 326 of the Labour Code)When you leave of your own accord, the law wants written notice — not a conversation and not an email. This document puts it in the form Art. 326(1) of the Labour Code requires, states the notice period and names your last working day, so there is no argument about when the contract ends. The period is thirty days unless your contract says otherwise, and by law it cannot exceed three months.
- Application to change working hoursFor moving to part-time hours, for a different start and finish, or for a flexible arrangement. The change affects an essential term of the employment contract, so it is put into effect by a written amendment — and part-time hours reduce pay proportionately. The application proposes a specific arrangement and the date it should start.
- Application for a student certificate or academic transcriptA student certificate proves that you are enrolled — needed for a grant, for reduced public transport fares, for a bank, for a visa, for deferral from military registration. A transcript lists the subjects studied and the marks, and is asked for on a transfer or when having qualifications recognised abroad. The two are not the same thing; say which you need and what for.
- Application to move a child to another kindergarten or schoolThe application to the head of the receiving institution. A move depends on available places and on that institution's own rules, so the application states where the child is coming from, into which group or class, and from which date. It is filed before the start of the term or the school year, when places are allocated.
- Consent to act as managing director and specimen signatureThe document by which a managing director accepts the post and gives the specimen signature the Commercial Register will hold. Its whole point is the notarisation — the signature is given in front of a notary, not at home. The lower part of the sheet is left blank for exactly that; do not fill it in.
- Loan for useFor when you lend out a thing free of charge — a tool, a piece of equipment, a car, a room. It differs from a lease in that nothing is paid, and from a money loan in that the same thing comes back rather than its value. So the description of the item and its condition at handover carry the whole weight of the contract: without them there is no way to show that what came back is not what went out.
- Annex to a contractThe general annex — it works for any contract in the catalogue. Amending a signed contract has to be done in writing, in the same form as the contract itself, and it must say which clauses change, with what wording, and from which date. An annex that states only what is new, without naming the clauses it replaces, is a common source of argument about what is actually in force.
- Service quotationThe first document a client sees, and the last one they come back to when there is an argument about what was promised. The quotation sets out the scope, the price and the timescale — and also how long it stays valid, without which old prices get produced months later. It states plainly what the price does not include, which heads off the most common falling-out at the start of a job.
- Objection to processing for direct marketingMarketing calls, messages and letters stop on an objection under Article 21 of the General Data Protection Regulation. Unlike the other rights, there is no balancing here and no exceptions: on an objection to direct marketing, processing for that purpose ends. The document lists the channels separately, because unsubscribing from the newsletter does not stop the phone calls.
- Application to terminate employment within the probationary periodWhile an agreed probationary period is running, the party in whose favour it was agreed may end the contract with no notice and without giving a reason. The application identifies the contract, the termination date, and that the ground is Art. 71 of the Labour Code. Check your own contract for whose benefit the probationary period was agreed before filing it.
- Commercial leaseFor a shop, a studio, a warehouse or an office. What sets it apart from a residential lease is purpose: the premises are taken for a particular activity, the permits for that activity are the tenant's, and any improvements the tenant makes have to be agreed in advance — otherwise they stay with the property and are not paid for. The contract settles all three separately.
- Notification of a claim event to the insurerWritten, dated notification of a loss. Insurers work with claim forms of their own, but filing those often drags — with this notice the date of notification is on record from the day you gave it. It describes the event and the property damaged, lists the documents drawn up, and says when an inspection can be carried out.
- Loan agreement between private individualsWhen you lend money to a relative or a friend, this is the document that turns a spoken arrangement into an obligation that can be proved. It records the amount in figures and in words, when it was handed over and how, when it is repaid, whether there is interest and what is owed on late payment. The amount in words is not a formality: where the two disagree, a court reads the words.
- Notice to terminate a tenancyThe written notice by which a landlord or a tenant brings a tenancy to an end. It works from either side — you state which of the two you are. The notice period is the one in your own agreement; the termination date runs from service, not from signature. Deliver it in a way that lets you prove when it was received.
- Application for exemption from physical educationExemption is granted by the head teacher and almost always rests on a medical document — an application with no certificate attached is usually sent back. Mark whether you are asking for a full exemption or only from particular activities, because a partial one is granted more readily. The pupil still has to attend the lesson unless the decision says otherwise.
- Application for a duplicate diplomaWhere a diploma is lost, destroyed or damaged, a duplicate is issued from the register held by the school or university. The application identifies exactly which diploma is wanted — year of completion, course, registration number if known — because the issuer searches an archive rather than a name. A fee is usually payable and a declaration of the circumstances required.
- Notice to terminate a service agreementEnding a retainer or an ongoing service needs written notice within the period the contract sets — otherwise the contract renews and the charges carry on. The notice identifies the agreement, the termination date and the notice period, and settles the three things that are argued about afterwards: the final payment, the materials to be returned, and the accounts and access that have to be closed.
