Request for a certificate of school enrolment and attendance (Italy)
An Italian request, in Italian, from a parent or an adult student to the school office for a certificate of enrolment or attendance for private use: an employer, a bank, a sports body, a foreign authority. Many schools issue certificates through the electronic register; with public offices, and now with private parties too, a self-declaration is enough. It has the space for the 16 € revenue stamp (sold at tobacconists; @e.bollo online), which is not applied where the use is exempt. Hand it in at the school office or send it by PEC.
Access to the civil invalidity assessment file (Italy)
An Italian request, in Italian, for access to documents (articles 22 ff. of Italian law 241/1990) sent to INPS or the local health authority to see or copy the civil invalidity, handicap or disability assessment (verbale) and the medical file of the procedure: for the person concerned, a parent or guardian, a delegate or an heir. The verbale can also be downloaded free from INPS’s online services. No stamp duty is due to view the file or for plain copies; certified copies need a 16 € stamp unless an exemption applies. Send it by PEC or by hand. Sold blank and filled in by hand.
Handing in a learning-disorder diagnosis and requesting a personalised learning plan (Italy)
An Italian letter, in Italian, from a parent or guardian to the head teacher handing in the diagnosis of a specific learning disorder (dyslexia, dysorthography, dysgraphia, dyscalculia) and asking for a personalised learning plan (PDP) with compensatory tools and dispensatory measures (Italian law 170/2010), and to take part in drafting it. Many schools have their own free form. Hand it in at the school office against a registration number or send it by PEC. It holds health data, so it is sold blank and filled in by hand.
Request for a copy of the medical record (Italy)
An Italian letter, in Italian, to the medical director’s office of a hospital or clinic asking for a copy of the medical record (cartella clinica) and other health documents: the patient, a delegate, a parent or guardian, or an heir gives the stay or treatment, the documents wanted and the format. The facility must hand them over within seven days (art. 4 of Italian law 24/2017). Many facilities have their own form or a free online service. Send it by PEC, registered post with proof of receipt, or by hand. It holds health data, so it is sold blank and filled in by hand.
Request for access to administrative documents (Italy)
An Italian request, in Italian, to a public body to see or copy documents of a procedure that concerns you, under articles 22 ff. of Italian law 241/1990: it names the documents, your interest and the type of access. The body answers within 30 days. Many bodies publish a free form. No revenue stamp is needed to view documents or for plain copies; certified copies need one (16 €, from a tobacconist or with @e.bollo), and there is space to affix it. Send it by PEC or hand it in at the office.
Request for clearance to transfer to another school (Italy)
An Italian letter, in Italian, from a parent or guardian to the head of the pupil’s current school asking for the nulla osta (clearance) to transfer the pupil to another school, also during the school year, and for the school records to be sent on. It gives the pupil, the class, the new school and the reason, with the other parent’s signature where parental responsibility is shared. Many schools have their own free form at the office or online. In compulsory schooling no stamp duty is due (Italian Presidential Decree 642/1972, table B, art. 11). Hand it in at the school office or send it by PEC.
Request to the Italian Revenue Agency to cancel an act in self-review (autotutela) (Italy)
For anyone who received from the Agenzia delle Entrate an assessment, a notice of irregularity or a tax bill with an obvious error: wrong person, calculation error, payment already made and ignored. Before this letter, the free route: the Agency’s CIVIS online service, for notices and bills, and its own fac-simile on plain paper. The request goes to the Agency office that issued the act or, for a tax bill, the one that entered the sums for collection, by PEC, recorded delivery or by hand, with no revenue stamp; it does not suspend the 60 days to appeal.
Request to the Comune to cancel an IMU, TARI or other local-tax act (Italy)
For anyone who received from the Comune, or the agent collecting for it, an assessment or payment notice for IMU, TARI or another local tax with an obvious error: a property not owned, wrong floor area or cadastral value, payment already made, wrong person. Many Comuni have their own form or a free online desk: if there is one, use it. This Italian request goes to the tax office that issued the act, by PEC or recorded delivery, and does not suspend the 60 days to appeal.
Domestic worker's request for payslips (Italy)
For a domestic helper, carer or nanny in Italy who asks the employing family for the payslips of certain months and for the statement of wages paid in the year, which the collective agreement for domestic work provides instead of the Certificazione Unica, as the family does not withhold tax. Art. 2120 of the Italian Civil Code allows it once during the employment, up to 70% of the TFR accrued, for exceptional medical costs or for buying a first home for oneself or a child; art. 7 of Italian Law 53/2000 adds costs during parental or training leave. The letter states the reason and encloses the documents. If the TFR goes to a pension fund, the advance is asked of the fund. Send it by registered post, PEC or by hand. In Italian.

For anyone who works or has worked as an employee in Italy and asks the employer, including a former one, for copies of the payslips for certain months and of the yearly tax certificates (Certificazione Unica) for certain years: for a mortgage, the NASpI unemployment benefit, a tax return or to check what was paid. Request to pay IMU, TARI and other municipal taxes in instalments (Italy)
For anyone who received from the Comune an executive assessment or another demand for IMU, TARI or another local tax and cannot pay it all at once. Many Comuni have their own free form: if there is one, use it. Instalments follow the Comune’s regulation or, failing that, Italian Law no. 160 of 27 December 2019: up to 72 monthly instalments depending on the amount, and a regulation must allow at least 36 for debts above € 6,000.01. The request declares a temporary financial difficulty and is exempt from stamp duty.

For anyone who paid an Italian income tax by F24 that was not due, or more than was due – a double payment, a wrong amount – and cannot recover it through the tax return. The request goes to the Agenzia delle Entrate office for the taxpayer’s tax domicile within 48 months of the payment (article 38 of Presidential Decree no. 602/1973). It is exempt from stamp duty. If the Agency stays silent for 90 days, an appeal can be made to the tax court (Corte di giustizia tributaria).

For an employee in Italy asking the employer for smart working, stating how much and from when, and, where it applies, the priority under art. 18(3-bis) of Italian Law 81/2017: parents of children up to twelve or with a disability, workers with a serious disability, carers. Smart working is not an automatic right: it is agreed in a written individual agreement. The letter asks for a written reply. Hand it over, send it by PEC or by e-mail. In Italian.
Claim for medical expenses to a supplementary health fund or insurer (Italy)
An Italian letter, in Italian, from a member of a supplementary health fund, a mutual fund or a health insurance policy, claiming back visits, tests, treatment or hospital stays already paid: it lists the invoices and prescriptions enclosed, gives the IBAN and asks to be told in writing of any missing documents. It is for funds that accept claims by post or PEC, or to chase a claim that has stalled; if the fund has its own members’ area or form, use those. It holds health data, so it is sold blank and filled in by hand.

The request, in Italian, asks the Region's tax office, or the body named on the notice, to cancel it on review; many Regions offer a free form or online service, to be preferred where it exists. In Friuli Venezia Giulia and Sardinia the tax is collected by the Agenzia delle Entrate, in Trentino-Alto Adige by the Province. No stamp duty; send by PEC, registered post or by hand. It does not stop the 60 days for an appeal.

For an employee in Italy asking in writing for a period of paid holiday, with the dates and the number of days, and for confirmation that it is granted. The letter recalls the statutory rules: the employer sets the period, taking account of the employee's interests (art. 2109 of the Italian Civil Code), and at least four weeks a year are due, two of them in the year they accrue, consecutive on request (art. 10 of Legislative Decree 66/2003). Hand it in, e-mail it or send it by PEC; if the company uses a leave portal, use that. In Italian.
