From when does the notice period run?

From its receipt by the other side, not from the date in the top corner. So it is served against signature or sent in a way that leaves a trace — in a dispute what is proved is the date of receipt, not of drafting.

Can I leave at once if the property is unfit?

Unfitness is a ground for a quicker exit, but it has to be established and communicated in writing rather than asserted on the way out. Give notice of the problem, allow a period to put it right, and only if it is not put right rely on this.

Do I owe rent during the notice period?

Yes — until the agreement has ended, the obligations under it continue. Vacating early does not shorten the period, unless the other side agrees in writing.

What should I do on the day I move out?

Draw up a handover record with the meter readings, hand over the keys against signature, and settle the deposit with a receipt. Moving out without those three is the reason most tenancy disputes exist at all.
