Notice to the employer of absence on sick leave (Bulgaria)
Why it matches: You are ill and off work, and you need to tell your employer in Bulgaria straight away in writing.
Bulgaria · in Bulgarian · for employees
A sick note is issued by the doctor, but the employer has to be told that you are absent — at once, not when you come back. This document does that in writing: the date your absence starts, the number of the sick note, and how to reach you. The notification does not replace the sick note; it goes with it.
Notice of absence on sick leave to the employer (Spain)
Why it matches: You are off sick in Spain and want to tell your employer in writing.
Spain · in Spanish · for employees
A Spanish letter, in Spanish, for an employee on sick leave in Spain who wants to tell the employer in writing. Since 1 April 2023 (Spanish Royal Decree 1060/2022) the employee no longer hands in a copy of the sick note: the INSS, Spain's social security institute, sends its details to the employer electronically. The letter only reports the absence, its start date and, if wished, the estimated length, never the diagnosis. Hand it over against a signed receipt, send it by certified post or burofax, or use the channel set by the collective agreement. It holds health data, so it is sold blank and filled in by hand.
Notice to the employer of leave under Law 104 (Italy)
Why it matches: INPS has approved your Law one hundred and four leave and you must tell your employer in Italy.
Italy · in Italian · for employees
For a private-sector employee in Italy whom INPS has granted the paid leave of art. 33 of Italian Law 104/1992, to care for a relative with a severe disability or for themselves. The application goes first to INPS, online and free, or through a patronato; this letter then gives the employer the details of the authorisation, with a copy enclosed, and the monthly schedule of days or hours, which the employer may ask for. The letter does not describe the disability. Send it by registered post, PEC or by hand. In Italian.

Why it matches: Your child is in hospital, you want to stay with them and tell your employer in writing.
Austria · in German · for employees
The notice telling an employer in Austria that you are taking care leave (Pflegefreistellung) under § 16 UrlG, the Austrian Holiday Act: to nurse a sick close relative or a sick member of your household, to look after your child when the person who normally cares for it is unavailable, or to stay with a child under ten in hospital. You choose whether this is the first week, the second week for a child under twelve who falls ill again or, once both are used, holiday taken without agreement (§ 16 Abs 3 UrlG). The employer keeps paying. No diagnosis. In German.
Notifying the employer of sickness (Germany)
Why it matches: You are ill and cannot go to work in Germany, and you need to let your employer know today.
Germany · in German · for employees
The written notice of sickness an employee in Germany gives the employer under § 5 of the Continued Remuneration Act (EntgFG): that they are unfit for work and for how long, with no diagnosis. For people with statutory health insurance it reflects the electronic sick note (eAU) — the employer fetches the certificate from the health insurer, but the employee must still report the absence at once. Those insured privately announce a paper certificate. In German, with a box for an address abroad.
Disciplinary dismissal letter for a household employee (Spain)
Why it matches: You employ someone to clean or care at home in Spain and want to dismiss them for serious misconduct.
Spain · in Spanish · for employers
For a householder in Spain who employs someone to work in the home (cleaning, childcare or elderly care) and dismisses them for a serious, culpable breach. The letter meets art. 55.1 of the Spanish Workers' Statute: the facts, the ground under art. 54.2, the effective date, the prior hearing and the final settlement. For domestic employment only. It is handed over against a receipt or sent by burofax. In Spanish.

The letter gives the prior notice of art. 37.3 of the Spanish Workers' Statute, states the reason and the dates and announces the supporting document. It is handed over against a receipt, sent by certified letter or by burofax. In Spanish.

Why it matches: Your doctor has signed you off sick in France and you need to send the employer’s part of the sick note.
France · in French · for employees
For an employee on sick leave in mainland France or the overseas departments other than Mayotte: the letter that goes with part 3 of the French sick note (avis d’arrêt de travail), the part meant for the employer. In French under French law, it says whether this is a first note or an extension, gives its dates and asks the employer to send the pay details to the health insurer. The note itself is the secured form the doctor completes. Sold blank, to complete by hand, as it holds health data DocMuse does not collect.
Notice of pregnancy to the employer (Italy)
Why it matches: You are expecting a baby and work as an employee in Italy.
Italy · in Italian · for employees
For an employee in Italy who is pregnant: the letter tells the employer of the pregnancy, with the protocol number of the electronic medical certificate and the expected date of birth, and the period chosen for maternity leave. The leave lasts five months: as a rule two before the expected date and three after the birth, or one and four, or all five after, with medical certificates; INPS pays 80% of pay, on an online application or through a patronato. The letter asks for the protections of Legislative Decree 151/2001: no risky or night work, paid time off for check-ups, no dismissal. In Italian.
Landlord's reminder of rent arrears (Austria)
Austria · in German · for landlords
The landlord's formal reminder to a tenant in Austria who is behind with the rent: the amounts owed for each month, the total, a deadline and the account. Under the Austrian Tenancy Act (MRG) a court notice to quit for arrears (§ 30 Abs 2 Z 1 MRG), like ending the lease under § 1118 of the Austrian Civil Code (ABGB), requires a reminder after the rent fell due that states the arrears specifically. You choose which step follows if the tenant does not pay. The court notice itself is not a letter but is filed at the district court — this is the reminder that must come first. In German.
Notice closing a current account (Germany)
Germany · in German · for consumers
A letter to a German bank or savings bank (Sparkasse) closing your current account (Girokonto), whether you are leaving altogether or switching to a new bank. It gives the IBAN, the account holder and date of birth, the closing date you want and the account for the remaining balance, ends the linked cards, overdraft and online banking, and asks for written confirmation. Under § 675h of the German Civil Code (BGB) you may close the account at any time without notice, unless a notice period of at most one month was agreed, and closing is free. A choice covers switching through the statutory switching service (§§ 20 ff. ZKG). In German.

Spain · in Spanish · for employees

It is handed over against a receipt, sent by certified letter or by burofax. In Spanish.
Notice naming the driver for a penalty-points fine (Italy)
Why it matches: The car belongs to your company and you have to say which employee was driving.
Italy · in Italian
The ticket usually comes with the police body's own free form for naming the driver, which can be used instead. This is the same notice as a letter in Italian, for the owner, the legal representative of a company or the lessee: the ticket's details, the driver's details and driving licence, with the driver's signed confirmation. It must be sent within 60 days of service (art. 126-bis of the Italian Highway Code), by registered post, PEC or by hand, even if you appeal or the owner was driving.

A letter by which an employee in Austria claims unpaid amounts from the employment in writing: wage or salary, overtime with its premium, special payments (13th and 14th salary) and other claims, with a total, a deadline and a bank account. It meets the forfeiture deadlines of the collective agreement (Verfallsfristen), which are often far shorter than the three-year limitation period (§ 1486 Z 5 ABGB), and refers to interest under § 49a ASGG. In German, to print, sign and deliver with proof of receipt.

Why it matches: Your partner is expecting and you want to give your employer advance notice of the Papamonat.
Austria · in German · for employees
The letter to an Austrian employer for the Papamonat, a father's month off at the birth under § 1a of the Fathers' Leave Act (VKG), in three versions: the advance notice at least three months before the due date, the notice of the start after the birth (within a week of it), and the notice after a premature birth that left no time for advance notice. Notice of illness during holiday (Austria)
Why it matches: You got sick on holiday abroad and need to tell your employer and send a doctor’s confirmation.
Austria · in German · for employees
An employee in Austria who falls ill on holiday does not lose the days: if the illness lasts more than three calendar days, the days of incapacity are not counted against the holiday entitlement (§ 5 UrlG, the Austrian Holiday Act). The employer must be told without delay after three days, and a doctor's confirmation shown on returning to work. This letter records both, with the holiday dates and the start and expected length of the incapacity, without a diagnosis — and a choice for illness abroad, where an official confirmation or a hospital's confirmation is needed. In German.
Final deadline for withheld pay before leaving early (Austria)
Why it matches: Your employer in Austria has not paid your wages and keeps putting you off.
Austria · in German · for employees

Leaving over unpaid wages without such a deadline has been held unjustified, so this letter is the first step. It also meets the forfeiture deadlines of the collective agreement.
