The PDF you download is in Italian — that is the language it has to be filed in, so it is not translated. Everything on this page is here to tell you what it says.

For an employee of an Italian ministry, region, municipality, health authority or other public body who is resigning. Public administrations are excluded from Italy's online resignation procedure: the employee writes to the administration, giving the notice set by the sector's collective agreement. School staff instead use the free application on the Istanze online portal. Sent by PEC, registered post or through the body's registry office. In the province of Bolzano the body must also accept German, in Valle d'Aosta French. In Italian.

Common questions

Does a public-sector employee in Italy resign through the online procedure?
No. The online procedure of art. 26 of Italian Legislative Decree 151/2015 does not apply to employment with public administrations: the resignation is sent in writing to the administration, by PEC to the address listed in the IPA index, by registered post or through its registry office. School teaching, educational and support staff instead resign with the free application on Istanze online, by the deadline the Ministry of Education and Merit sets each year.
What notice must a public-sector employee give when resigning in Italy?
The notice in the national collective agreement of the sector (central government, local government, health and so on), which varies with length of service; the personnel office will state it. A resignation is a unilateral act: no acceptance is needed, but the administration records it and sets the end date. If the notice is not respected, the administration may withhold the corresponding indemnity.
Can a resignation to an Italian public body be written in German or French?
In the province of Bolzano German has equal standing with Italian before the public offices located there (Presidential Decree 574/1988), and in Valle d'Aosta French does (art. 38 of the Special Statute): the body must accept the letter in that language too. This form is in Italian and valid throughout Italy.

It is for cases where the collective agreement or the company asks for written notice and offers no portal of its own. It states term, duties, the statutory trial-period formula, pay and the right of precedence. The term counts only if written; the copy is due within five working days. In Italian.

The letter does not describe the disability. Send it by registered post, PEC or by hand. In Italian.
