Notice of withdrawal from a distance contract (Poland)
A consumer's notice withdrawing, without giving reasons, from a contract made online, by phone or away from business premises in Poland (art. 27 of the Consumer Rights Act): what was bought, the order number, when the contract was made and the goods received, and a demand for every payment back, delivery included, within 14 days. It covers what the statutory model form does and reminds the reader of the deadline for sending the goods back.
Notice of price reduction or withdrawal for non-conforming goods (Poland)
A consumer's notice for when the seller refused to repair or replace goods that do not conform to the contract, did not do it in reasonable time, the repair did not work, or the defect is too serious to wait (art. 43e of the Consumer Rights Act): the choice between reducing the price and withdrawing from the contract, the amount and the account for the refund. It reminds the seller to repay within 14 days and to take the goods back at its own cost.
Complaint about goods that do not conform to the contract (Poland)
A consumer complaint under the Polish rules on goods not conforming to the contract, which replaced the Civil Code warranty between shops and consumers on 1 January 2023: the item, when it was bought and for how much, the proof of purchase, what is wrong and when it showed, and a demand for repair or replacement. It reminds the seller that it is liable for 2 years from delivery and that no answer within 14 days means the complaint is accepted.

A mutual NDA for two businesses in talks about cooperation, an investment or a deal: the purpose, a definition of confidential information linked to trade secrets under Polish law, the exceptions, rules on access and protection, return of materials, how long it lasts after the talks, and a contractual penalty with the right to claim damages above it. It creates no obligation to sign anything further.
Contract for a specific work (umowa o dzieło)
A Polish contract for a specific, verifiable result — a design, a text, software, an object: the work and its requirements, the deadline, a lump-sum fee in words, acceptance by protocol, remedying defects and warranty, the right to withdraw for delay, and transfer of copyright on the named fields of exploitation. It reminds a business client to report the contract to ZUS on the RUD form.
Pre-action demand for payment between businesses (Poland)
A creditor business's demand to a trading partner for overdue invoices: the principal, the list of invoices, statutory late-payment interest for commercial transactions and the fixed compensation for recovery costs — the equivalent of 40, 70 or 100 euro per payment — the deadline and the account. It is the attempt at amicable settlement the court asks about when a claim is filed (art. 187 § 1(3) of the Code of Civil Procedure).
Minutes of an annual shareholders' meeting with resolutions (Poland)
Minutes of the annual shareholders' meeting of a Polish limited company with the three resolutions art. 231 of the Commercial Companies Code requires: approving the financial statements and the management report, distributing profit or covering a loss, and discharging the board members. It records the chair's election, the meeting's capacity to pass resolutions, the agenda and the votes. The meeting must be held within 6 months of the financial year's end.
Service agreement between businesses (umowa o świadczenie usług)
An agreement between Polish businesses for ongoing or periodic services — IT, marketing, accounting, consulting: the scope and what falls outside it, the term, the net fee plus VAT, payment within no more than 60 days, invoices in the national e-invoicing system (KSeF), professional diligence, confidentiality, a liability cap and termination. The rules on mandate contracts apply to it (art. 750 of the Civil Code).
Mandate contract (umowa zlecenia)
A Polish mandate contract with an individual: the tasks, period and place, the contractor's independence, an hourly rate no lower than the statutory minimum — 31.40 zł gross in 2026 — confirmation of hours, monthly payment, a statement of social-insurance status, and termination. A mandate contract generally carries social-security contributions, which is what sets it apart from a contract for a specific work.
Management board resolution changing a company's address (Poland)
A resolution of the management board of a Polish limited company (sp. z o.o.) moving its address within the town of its registered seat: the resolution number, company name and KRS number, the new address, the effective date, the vote and the board members' signatures. The articles name only the town, so no shareholders' meeting or notary is needed — but the change must be filed with the KRS within 7 days, and only electronically.
Shareholders' resolution appointing a management board member (Poland)
A resolution of the shareholders' meeting of a Polish limited company appointing a management board member: the person's details and PESEL, the role, the term, the effective date, the secret ballot and its result, and the chair's and minute-taker's signatures. It notes that the change is filed with the KRS within 7 days together with the appointee's consent and address for service — the statement for that is a separate document in the catalogue.
Consent to appointment to a management board, with address for service (Poland)
The statement of a person appointed to a Polish company's management board, attached to the KRS filing: consent to the appointment and the role, the address for service required by art. 19a of the National Court Register Act, and a statement that none of the bars in art. 18 of the Commercial Companies Code applies. Consent is not needed where the person signs the filing personally or consent is recorded in the minutes of the appointing body.
Application to annul a final fixed-penalty ticket (Poland)
An application to the Polish district court to annul a fixed-penalty ticket (mandat) that has already been accepted — for a traffic offence, say — under art. 101 of the Code of Procedure for Petty Offences. It identifies the ticket, the date it became final, the statutory ground and the reasons. The grounds are narrow — an act that is not an offence, a person under 17, a fine above the legal limit — and the deadline is 7 days from the ticket becoming final. The document says so plainly.
Authorisation to drive someone else's vehicle (Poland)
A car owner's authorisation for another person to drive it for a set time and in a set territory — for a trip abroad, or when lending the car to family or an employee. It gives both people's details, the vehicle, the OC policy, what is authorised, and states plainly that selling or passing the car on is not. There is room below the signature for notarisation, which some non-EU countries require.
Claim against the at-fault driver's motor insurer (Poland)
The injured party's claim to the at-fault driver's insurer: the date and place, the at-fault driver, their vehicle and policy number, the injured party's vehicle, what happened, the damage, the evidence, the amount claimed and the account for payment, reserving the right to a replacement-car hire refund. It reminds the insurer that compensation is paid within 30 days of the claim, and filing the claim interrupts the limitation period.
At-fault driver's statement after a collision (Poland)
A statement written at the scene of a minor collision in Poland where nobody was hurt: the date, time and place, the at-fault driver's details and licence, their vehicle and OC policy, the other driver and vehicle, what happened, visible damage, witnesses, room for a sketch, and the admission of fault. It is the main evidence for a claim against the at-fault driver's insurer when the parties do not call the police.
Notice to the insurer that a vehicle was sold (Poland)
The seller's letter to the insurer that a car with OC motor insurance has been sold: the policy number, the vehicle, the date of sale and the buyer's details that art. 32 of the Compulsory Insurance Act requires. Since 10 September 2025 the deadline is 14 days, and until the insurer is told the seller is jointly liable for the premium with the buyer. It does not replace the notice to the district office.
Car sale agreement (umowa kupna-sprzedaży samochodu)
A Polish agreement for the private sale of a used car: the parties, make and model, VIN, registration number, mileage, known defects, the price in figures and words, and handover with the documents and keys. It also sets out what each side must do afterwards: the seller tells the district office within 30 days and the insurer within 14, the buyer registers the car within 30 days and pays 2% tax on civil-law transactions within 14. No notary is needed.
Freedom of information request (wniosek o udostępnienie informacji publicznej)
A request under Poland's Access to Public Information Act of 6 September 2001, to an office, municipality, school, municipal company or other body performing public tasks: the information sought, the form and way it should be provided, and a reminder of the 14-day deadline and that no legal or factual interest need be shown. No answer in time opens the way to a complaint about inaction to the regional administrative court.
Appeal against a tax decision (odwołanie od decyzji podatkowej)
An appeal against a first-instance decision of a Polish tax office head, mayor or other tax authority, with what art. 222 of the Tax Ordinance requires: the objections to the decision, the substance and scope of the demand, and the evidence. It is filed within 14 days of the decision being served, through the authority that issued it. The appeal is free, and the appeal body cannot leave the appellant worse off unless the decision flagrantly breaches the law or the public interest.
Application to pay tax arrears in instalments (Poland)
An application to a Polish tax office or other tax authority to spread tax arrears and interest over instalments (art. 67a § 1(2) of the Tax Ordinance): the tax and period, the amount, the proposed number of instalments and schedule, and above all the taxpayer's important interest that justifies it, with the household's income and outgoings. The relief is discretionary, so a thorough justification decides the outcome.
Application to change a first name or surname (Poland)
An application to a Polish civil registry office (urząd stanu cywilnego) for an administrative change of first name or surname, with what the Act on changing names requires: the applicant's details and birth surname, the offices holding the birth and marriage certificates, the new name, any children it extends to, the important reason, and a statement that no application in the same matter was made elsewhere and no refusal was issued. It can be filed at any civil registry office.
Cohabitation agreement for unmarried partners (Poland)
An agreement for couples living together in Poland without marrying: how they share housing and living costs, what each keeps as personal property, how things bought together are co-owned, how they settle up if they separate, and who leaves the home. Poland has no registered partnerships — the closest-person status law did not take effect after the Sejm failed to override the presidential veto in September 2026 — so these matters are settled by a civil-law contract.
Parental consent to a passport for a child (Poland)
A parent's written consent to a Polish passport being issued to a minor, with the details art. 14 of the Passport Documents Act requires: the parent's name, PESEL number and identity document, and the child's name and PESEL. On paper it counts only once the signature is certified by a notary or a passport authority — a voivode's office or a consul; the document leaves room for that.
