Authorisation to register at a home on the padrón (Spain)
Registering on the padrón, Spain's municipal register of residents, is free at your town hall, in person or on its electronic office, with its own form (hoja padronal); you do not need DocMuse for it. This Spanish document, in Spanish, is only the authorisation many town halls ask for when the person registering has no contract in their name: it is signed by someone already registered at the home who holds a right over it or, if nobody is registered, by the home's holder, and names each person authorised. Hand it in at the town hall with the form and the copies it asks for.
Administrative appeal (alzada or reposición) against a decision (Spain)
You can file the appeal free of charge on the body's electronic office, with its own appeal form or general application form; you do not need DocMuse for it. This Spanish document, in Spanish, is for challenging an act of a Spanish ministry, regional government or town hall: the recurso de alzada to the higher body, if the act does not end the administrative route, or the recurso de reposición to the same body, if it does. The notification says which applies, to whom and the time limit: one month for an express act. It follows art. 115 of Spanish Law 39/2015; you write the grounds. Not for taxes or traffic fines. File it through a registry.
Request for a school enrolment or academic certificate (Spain)
A Spanish written request, in Spanish, for an adult student, or a parent or guardian of a student in Spain, who needs a certificate from the school: of enrolment in the current year, of the marks obtained, or of the studies completed. It is addressed to the school office or the head, states what the certificate is for and how it is to be delivered. In some Spanish regions the enrolment certificate can be downloaded free from the regional education platform. Hand it in at the school office against a stamped copy, or file it through a public registry.
Payment reminder for an overdue invoice (Spain)
For a self-employed person or business in Spain with an overdue unpaid invoice: a courteous Spanish reminder identifying the invoice, its amount and due date, asking for payment or for proof if already paid, and recalling that late-payment interest and recovery costs under Spain's Law 3/2004 run from the due date. Sent by e-mail or, for proof of content, by burofax; if payment does not follow, the next step is the demand with an invitation to negotiate. In Spanish.
Acknowledgement of debt (Spain)
For someone in Spain who owes money to a private person or a company and acknowledges it in writing, or for the lender who wants that paper signed. The debtor, identified by their DNI or NIE, acknowledges the amount in words and figures and where it comes from, and undertakes to pay it in one sum, by bank transfer, by a set date. The acknowledgement interrupts limitation, and the five-year period starts again (articles 1964 and 1973 of the Spanish Civil Code). The debtor signs it and hands the original to the creditor. In Spanish.
Complaint about a contract you never asked for or a switch without consent (Spain)
For a person in Spain whom an electricity, gas, phone or other company has signed up, switched from their supplier or charged for a service they did not ask for. The letter denies any consent, recalls that silence is not acceptance and that unsolicited supplies need not be paid for (art. 66 quáter of the Spanish Consumer Protection Act, TRLGDCU), asks for the contract to be cancelled, a return to the previous company and a refund, and invites the company to negotiate as Spanish Organic Law 1/2025 requires. Send it by burofax with certified content. In Spanish.
Debt reduction and deferral agreement (Spain)
For a creditor and a debtor in Spain who agree to reduce a debt (quita) and give more time to pay it (espera). It is a form with fields: the original debt in words and figures, the reduction, the amount left to pay, the schedule in one payment or in instalments, the account, and the days of delay allowed before the reduction is lost. Once the agreed sum is paid the debt is extinguished and a receipt in full settlement is given. It records the negotiation under Organic Law 1/2025 and may be made into a notarial deed. Signed in two copies. In Spanish.
Payment or debt reduction proposal to a creditor (Spain)
For someone in Spain who owes money to a bank, a lender, a utility or a debt collector and wants to offer a smaller lump sum (quita) or instalments (espera) to close the debt. The letter states the amount claimed and the amount offered, the schedule, and makes everything subject to written acceptance stating that, once paid, the debt is settled, with a receipt in full and removal from bad-debtor files. Nothing is paid before that acceptance. Send it by burofax or through the creditor's written channel. In Spanish.
Notice of absence on sick leave to the employer (Spain)
A Spanish letter, in Spanish, for an employee on sick leave in Spain who wants to tell the employer in writing. Since 1 April 2023 (Spanish Royal Decree 1060/2022) the employee no longer hands in a copy of the sick note: the INSS, Spain's social security institute, sends its details to the employer electronically. The letter only reports the absence, its start date and, if wished, the estimated length, never the diagnosis. Hand it over against a signed receipt, send it by certified post or burofax, or use the channel set by the collective agreement. It holds health data, so it is sold blank and filled in by hand.
Complaint to a bank's customer service department (Spain)
For a customer of a bank in Spain with a problem over fees, charges, cards, transfers or any other product who wants to complain in writing. It is the compulsory first step: the customer service department has fifteen working days for payment services and one month otherwise; if it does not reply or rejects the complaint, the next step is a complaint to the Banco de España, on its own form. The letter sets out the facts, states what is requested and invites negotiation (Organic Law 1/2025). Hand it in at the branch against a stamped copy, use the bank's online channel, or send it by burofax. In Spanish.
Claim for reimbursement of medical expenses from a health insurer (Spain)
A Spanish letter, in Spanish, for a person covered by a private health insurance policy with expense reimbursement in Spain who has paid for a consultation, a test or a treatment and wants back what the policy covers. It claims the refund from the insurer under Article 105 of the Spanish Insurance Contract Act, lists the care, the invoices, the amount and the IBAN, and asks for any refusal to be explained in writing. Send it by certified post, burofax or the insurer's customer area. It holds health data, so it is sold blank and filled in by hand.
Request to cancel a phone, internet or TV contract (Spain)
For the holder of a mobile, landline, broadband or TV line in Spain who wants to cancel and not be charged any more. The letter tells the operator to end the services two working days after receiving it, asks for the reference number, the closing bill and instructions for returning the equipment, and that nothing be charged afterwards. Send it by burofax with certified content, which proves the date from which the operator may no longer bill; you can also use any channel the operator offers. In Spanish.
Claim to the Transparency Council over refused information (Spain)
This can be done free of charge on the electronic office of Spain's Transparency and Good Governance Council, with its own form, or with your region's transparency body; you do not need DocMuse for it. This Spanish document, in Spanish, is for people who prefer to file through a registry: it challenges the full or partial refusal of a request for public information, or the lack of any answer, on your own grounds, within the month set by art. 24.2 of Spanish Law 19/2013. The Council has three months to decide.
Request for access to public information (Spain)
This can be done free of charge on the Spanish State's Transparency Portal and on each body's electronic office, with their own form; you do not need DocMuse for it. The body has one month to decide.
Complaint to an insurer about a claim or a policy (Spain)
For an insured person or policyholder in Spain whose insurer refuses or delays paying a claim, pays less than it owes, overcharges or does not honour the policy. The letter files the complaint in writing with the insurer's customer service or customer ombudsman, a required step before the free complaints service of Spain's Directorate-General for Insurance and Pension Funds (DGSFP), and includes the invitation to negotiate that Spanish Organic Law 1/2025 requires before suing. Send it by burofax with certified content. In Spanish.
Complaint to a Spanish consumer office (OMIC) with a request for arbitration (Spain)
This step is free on your town hall's or region's online office in Spain; you do not need DocMuse for it. For a consumer who already complained to the company and got no reply or no solution: a written complaint to the municipal consumer office (OMIC) or the regional consumer service asking for mediation and, if you choose, consumer arbitration. File it at the office, at any Spanish public registry or at a Correos post office as «correo administrativo». In regions with a co-official language the office must also accept it in that language. In Spanish.
Request for access to and a copy of the medical record (Spain)
A Spanish letter, in Spanish, for a patient who wants to see and copy their medical record in Spain, for their representative, or for a relative of a deceased patient. It exercises the right of access under Article 18 of Spanish Law 41/2002 and Article 15 of the GDPR before a hospital, health centre or private clinic: it names the documents, the period and how the copy is to be delivered, and recalls the one-month deadline and that access is free. Hand it in at the patient or admissions service or send it by certified post; to a public centre, also through a public registry. It holds health data, so it is sold blank and filled in by hand.

It includes the invitation to negotiate that Spanish Organic Law 1/2025 requires before going to court. Send it by burofax with certified content, which proves what was sent and when. In Spanish.
Request for removal from a bad-debtor file (Spain)
For someone listed in Spain in a bad-debtor file such as ASNEF or Experian for a debt that is paid, does not exist, is disputed, was never notified, is over five years old or is under fifty euros. The letter exercises the right to erasure against the company running the file, asks for processing to be restricted while it checks, and points out the one-month deadline to answer (article 20 of Spain's data protection act, LOPDGDD, and articles 12 to 19 GDPR); a copy goes to the creditor. Exercising these rights is free, and the AEPD offers its own model forms. Send it by burofax or through the file's written channel. In Spanish.

It is addressed to the patient service (servicio de atención al paciente), asks for the facts to be looked into and for a reasoned answer within the period set by the region's rules. It is not a claim for compensation. File it at the service itself, at any public registry office, at a post office or online. It holds health data, so it is sold blank and filled in by hand.
