Find the document you need
Answer three short questions. You don't need to know what the form is called.
1. Which country is it for?
2. What is it about?
3. What do you want to do?
32 documents match
- Request for erasure of personal dataThe right to be forgotten under Article 17 of the General Data Protection Regulation. It is not unconditional: data the organisation keeps by law, for accounting, or to defend a legal claim stay. So the request names exactly which data and on what ground, and asks for a written answer — including when the answer is a refusal, so that there is something to appeal against.
- Application to change working hoursFor moving to part-time hours, for a different start and finish, or for a flexible arrangement. The change affects an essential term of the employment contract, so it is put into effect by a written amendment — and part-time hours reduce pay proportionately. The application proposes a specific arrangement and the date it should start.
- Application for a student certificate or academic transcriptA student certificate proves that you are enrolled — needed for a grant, for reduced public transport fares, for a bank, for a visa, for deferral from military registration. A transcript lists the subjects studied and the marks, and is asked for on a transfer or when having qualifications recognised abroad. The two are not the same thing; say which you need and what for.
- Application to move a child to another kindergarten or schoolThe application to the head of the receiving institution. A move depends on available places and on that institution's own rules, so the application states where the child is coming from, into which group or class, and from which date. It is filed before the start of the term or the school year, when places are allocated.
- Application for exemption from physical educationExemption is granted by the head teacher and almost always rests on a medical document — an application with no certificate attached is usually sent back. Mark whether you are asking for a full exemption or only from particular activities, because a partial one is granted more readily. The pupil still has to attend the lesson unless the decision says otherwise.
- Application for a duplicate diplomaWhere a diploma is lost, destroyed or damaged, a duplicate is issued from the register held by the school or university. The application identifies exactly which diploma is wanted — year of completion, course, registration number if known — because the issuer searches an archive rather than a name. A fee is usually payable and a declaration of the circumstances required.
- Parent's application for a pupil's absenceThe application by which a parent excuses a child's absence for family reasons. How many days a parent may excuse themselves, and from which point the head teacher's permission is needed, is set by each school's own rules — check yours before filling in the dates. It is filed in advance, not after the child is back in class.
- Request for a certificate of incomeThe bank, the court or the social services office wants a document showing your income, and the employer wants a written request. This form is that request: for which period, for what purpose, in how many copies. Stating the purpose matters — a certificate for a loan and a certificate for social assistance often carry different figures.
- Freedom of information requestEvery institution is obliged to answer a request like this within fourteen days. The form follows Art. 24 of the Access to Public Information Act and carries every particular the Act requires, so that it cannot be set aside without consideration.
- Application to reduce or waive a feeThe grounds differ from one school and university to the next — financial circumstances, outstanding results, being orphaned, a disability, a large family — but the procedure is the same: a written application with documents attached, filed in time and before the fee is paid. Once the fee has been paid the request becomes a claim for a refund, which is slower and refused more often.
- Application for after-school provisionA place in after-school provision is at the parent's request and is applied for in writing, usually before the school year starts, because whether a group is formed at all depends on how many applications come in. The application states from which date, until what time, and who will be collecting the child — the last matters, because a teacher will not release a child to anyone not on the list.
- Subject access requestThe right under Article 15 of the General Data Protection Regulation: to find out what data an organisation holds about you, where it got them, who it gives them to and how long it keeps them. The request is free and goes straight to the controller, not to the regulator. Word it specifically — a blanket request for everything usually comes back with the least that will do.
- Request for a reference from an employerA reference is not a certificate: it is an assessment of your work, and an employer is not obliged to give one. So the request states which period, for what purpose and in which language you need it — a specific request is answered far more often than a general one.
- Application to terminate employment by mutual consent (Art. 325(1)(1) of the Labour Code)The way to leave at once, without working a notice period — but only if the employer agrees. The application proposes a date on which the contract is to end. The part almost nobody knows: the employer must answer within seven days, and silence counts as a refusal. Without written consent there is no termination.
- Leave applicationA standard application to an employer for paid, unpaid or any other kind of leave, stating the period and the number of working days it covers.
- Application to transfer to another course or mode of studyA transfer depends on available places and on how many of the examinations already passed will be recognised — which is why this application expressly asks for recognition of the subjects and a plan for making up the difference. Without that question a student transfers and then discovers they owe ten examinations. Moving from full-time to part-time study also changes the fee and the entitlement to a hall place and a grant.
- Application to sit a resit examinationSitting a resit is applied for in advance and within the deadline — being entered on the examination record follows from an application, not from turning up in the hall. Give the subject, the examiner and the session exactly as they appear in the curriculum, because a mismatch is the commonest reason an application comes back. Check too whether a fee is payable.
- Application to interrupt an academic yearInterrupting studies is granted on particular grounds — health, family, work abroad, maternity — and almost always requires a document. What the application asks expressly is which examinations remain outstanding and how they are to be made up, because on returning a student is reinstated into a specific year, not where they remember being.
- Request to convene a general meeting of the owners' associationWhen the manager does not call a meeting, the owners can ask for one in writing. The request has to carry the signatures of enough owners and to set out a ready agenda — a meeting cannot decide on items that were not announced. If the manager does not call the meeting in time, those who asked for it may call it themselves.
- Request for a copy of the employment contract and its amendmentsYou need it when applying for a loan, in a dispute about a post or about pay, or simply because your own copy is lost. The employer keeps the personnel file and issues certified copies of what is in it. The request lists the documents you are after by name, because a generally worded one usually comes back with a single sheet.
- Application for leave on marriage, blood donation or bereavementPaid leave under Art. 157 of the Labour Code is due on specific occasions — getting married, donating blood, the death of a close relative, attending court. It is separate from annual leave and is not deducted from it. The number of days is set by the Act and may be increased by a collective agreement, which is why it is a field here rather than an assumption.
- Application to withdraw a notice of resignationA notice cannot be withdrawn unilaterally — once it has been given, withdrawing it needs the employer's agreement. That is why this document is an application rather than a notification, and why it should be filed as early in the notice period as possible. It identifies the notice by date and asks expressly for written confirmation that the employment continues.
- Application to work remotelyRemote work is not settled by a spoken arrangement — it is written into an amendment to the employment contract, which fixes the place of work, the working hours and who provides the equipment. This application puts the request in writing and proposes a specific arrangement, rather than leaving everything to be decided afresh the next time management changes.
- Application for unpaid leaveUnpaid leave is not an entitlement — it is granted by the employer, so the application has to be in writing and has to carry a decision on it. State the exact period and the reason, because whether the time counts towards length of service and insurance depends on how long it is. File it early enough for someone to be able to cover your work.
