Why it matches: You want to work from home some days a week and your employer in Greece agrees.
Greece · in Greek
The agreement by which a Greek employer and employee set up telework, at hiring or as an amendment to the contract (art. 67 of Law 4808/2021): the pattern and place of telework, working hours and time recording, equipment and costs, the right to disconnect and the ban on camera monitoring. The law puts the cost of equipment, telecoms and repairs on the employer, unless use of the employee's own equipment is agreed, and wants the terms in writing within eight days.
Papeleta de conciliación: request for conciliation after a dismissal (Spain)
Why it matches: Your employer in Spain has dismissed you and you disagree with the dismissal.
Spain · in Spanish · for employees
For an employee in Spain dismissed by a company or private employer who disagrees with the dismissal. The papeleta de conciliación is the compulsory first step before suing: it is filed with the region's mediation, arbitration and conciliation service within twenty working days of the dismissal (Art. 59.3 of the Workers' Statute; Art. 63 and 103 of the Labour Procedure Act), and it suspends that period. It works where the service accepts a request you write yourself; Andalusia, for example, requires its own form. Without agreement, the claim goes to the labour section of the Tribunal de Instancia. In Spanish.
Notice of eight-week parental leave (Spain)
Why it matches: You want to give your employer the dates of your parental leave in writing.
Spain · in Spanish · for employees
For an employee in Spain with a child under eight who wants to take the parental leave of art. 48 bis of the Spanish Workers' Statute, of up to eight weeks. The letter tells the employer the weeks, the start and end dates and whether it will be full-time or part-time, with the ten days' notice the law sets or the collective agreement's. It is handed over against a receipt, sent by certified letter or by burofax. In Spanish.
Paid leave request (France)
Why it matches: You want to ask your employer in France for paid holiday on specific dates.
France · in French · for employees
A paid-leave request from an employee to the employer in mainland France or an overseas department other than Mayotte: first and last day, the number of days counted in jours ouvrables or jours ouvrés, the date of return, and room for the employer's signed agreement. Leave is earned at two and a half jours ouvrables per month worked (article L. 3141-3 of the French Labour Code); the employer sets the order in which staff take it. An e-mail often does; the letter keeps a written record of the agreement. In French.
Request for unpaid leave – Karenzierung (Austria)
Why it matches: Your employer has agreed in principle to unpaid leave and you want the terms in writing.
Austria · in German · for employees
A request to an Austrian employer for unpaid leave (Karenzierung), with the agreement both sides sign below it: the period, the reason, the return to work, the collective agreement and what applies to annual leave and special payments. There is no right to unpaid leave in Austria — it is agreed. The letter records that compulsory social insurance continues only for unpaid leave of up to one month (§ 11(3)(a) of the Austrian General Social Insurance Act, ASVG). No form is prescribed. In German, ready to print and sign.
Letter claiming unpaid wages, overtime and special payments (Austria)
Why it matches: Your employer in Austria has not paid your full salary or wages.
Austria · in German · for employees
A letter by which an employee in Austria claims unpaid amounts from the employment in writing: wage or salary, overtime with its premium, special payments (13th and 14th salary) and other claims, with a total, a deadline and a bank account. It meets the forfeiture deadlines of the collective agreement (Verfallsfristen), which are often far shorter than the three-year limitation period (§ 1486 Z 5 ABGB), and refers to interest under § 49a ASGG. In German, to print, sign and deliver with proof of receipt.
Unpaid leave request (Germany)
Why it matches: Your employer is open to unpaid leave and you want the terms agreed in writing.
Germany · in German · for employees
A request to a German employer for unpaid special leave (unbezahlter Sonderurlaub) over set dates, giving the reason and the terms both sides agree: release from work without pay, the employment continuing, and a return on the same terms. There is no general statutory right to it, so the leave exists only once the employer agrees, and the document has room below for that consent. It points out that social insurance cover continues for one month at most (§ 7(3) of the German Social Code, Book IV, SGB IV), after which health insurance must be arranged. In German.

Why it matches: You want to ask your employer for Bildungsurlaub, also called Bildungszeit.

Bavaria has no educational leave, and Saxony only from 1 January 2027. In German.

Why it matches: You want a longer break for a family situation, a course or a trip, and your employer may agree.
Romania · in Romanian · for employees

The maximum length is set by the collective agreement or the internal rules.
Annual leave request (wniosek o urlop wypoczynkowy) (Poland)
Why it matches: You are planning a holiday and want to ask your employer in Poland for the days off.
Poland · in Polish · for employees

Leave is 20 days with under 10 years' service and 26 days with at least 10 years, and time in education counts towards that. Leave on demand — up to 4 days a year — is requested on the day it starts at the latest.
Application for parental (childcare) leave (Bulgaria)
Why it matches: Your employer wants a written application before your leave to raise your child can begin.
Bulgaria · in Bulgarian · for employees
Leave under Art. 164 of the Labour Code is taken after maternity leave and is granted on an application to the employer, while the benefit is paid by the social security institute through a separate procedure. The age limit and the amount of the benefit are set by law and do change — check them for the year you are applying in. The application also states whether the leave is being transferred to the other parent or to a grandparent, which is common.
Remote working agreement (Spain)
Why it matches: As an employer, you want to settle who pays for the laptop, internet and power of someone working from home.
Spain · in Spanish
For an employer and employee in Spain agreeing regular remote work — at least thirty per cent of working time over three months —, which Spanish Law 10/2021 requires in writing before it starts. It covers the minimum content of art. 7: inventory of equipment and its useful life, expenses and how they are compensated, hours and availability, percentage and days, the workplace the employee belongs to and the chosen place, notice to return to the office, monitoring, technical problems, data and duration. Both sign; the employer gives the representatives a copy within ten days. In Spanish.

Why it matches: You want to take your summer holiday from work in Austria and agree the dates with your employer.
Austria · in German · for employees
A request to an Austrian employer to agree the dates of your annual leave under § 4(1) of the Austrian Leave Act (UrlG): the period, the number of days — counted as Werktage (Monday to Saturday) or as working days, as your workplace counts them — the leave year and who covers for you. In Austria leave is agreed, not granted: below the letter, both sides sign the leave agreement or record different dates. No form is prescribed; the signed sheet proves the agreement. In German, ready to print and sign.
Papamonat: advance notice and notice of a father's month off (Austria)
Why it matches: Your partner is expecting and you want to give your employer advance notice of the Papamonat.
Austria · in German · for employees
The letter to an Austrian employer for the Papamonat, a father's month off at the birth under § 1a of the Fathers' Leave Act (VKG), in three versions: the advance notice at least three months before the due date, the notice of the start after the birth (within a week of it), and the notice after a premature birth that left no time for advance notice. With due date, start and end of the leave and a receipt block. The Papamonat is unpaid; the family time bonus (Familienzeitbonus) is claimed separately from the health insurer, usually the ÖGK. In German.

Why it matches: You want to give an employee or volunteer the written request for an erweitertes Führungszeugnis.
Germany · in German · for employers
The written request a club, nursery, school, youth-welfare provider or other organisation in Germany gives an employee or volunteer so that they can apply for an extended certificate of good conduct (erweitertes Führungszeugnis): under § 30a(2) of the Federal Central Criminal Register Act (BZRG), it is issued only to someone who presents such a request, in which the organisation confirms that the conditions of § 30a(1) are met. It names the organisation, the person and the activity, gives the statutory ground as a choice and says whether the work is voluntary, for a fee waiver. In German.
Annual leave request (Germany)
Why it matches: You want to take holiday from work in Germany on specific dates and need to ask your employer.
Germany · in German · for employees
A request to a German employer for annual leave (Erholungsurlaub) over set dates, with the number of working days and who covers the work, citing § 7(1) of the Federal Leave Act (BUrlG): the employee's wishes must be followed unless urgent operational needs, or colleagues with a socially stronger claim, stand in the way. It says the leave starts only once approved, and leaves room below for the employer's decision. No form is prescribed: e-mail it or hand it in. In German.
Parental leave request (wniosek o urlop rodzicielski) (Poland)
Why it matches: You want to hand your employer a parental leave request with the dates and the length.

Request for UP-2 and UP-3 certificates (Bulgaria)
Why it matches: You need certificates of your insured earnings and length of service from a former employer.
Bulgaria · in Bulgarian · for employees
These certificates are requested from an employer when retiring or when proving length of service: UP-2 shows insurable income, UP-3 the length and category of service. They are two different documents, issued separately, so mark which of them you need. If the employer no longer exists, they are sought from the regional archive rather than from the employer.
Application for leave on marriage, blood donation or bereavement (Bulgaria)
Why it matches: You are getting married and want the paid days off for your wedding from your employer in Bulgaria.
Bulgaria · in Bulgarian · for employees
Paid leave under Art. 157 of the Labour Code is due on specific occasions — getting married, donating blood, the death of a close relative, attending court. It is separate from annual leave and is not deducted from it. The number of days is set by the Act and may be increased by a collective agreement, which is why it is a field here rather than an assumption.
