Parent's request to excuse a pupil's absences (Romania)
The request in which a parent in Romania asks for a child's absences for family reasons to be excused: the period, the number of hours and the reason. The framework regulation for pre-university schools (ROFUIP) allows absences to be excused on a parent's written request approved by the head, for no more than 40 hours a school year and no more than a fifth of one subject's hours, and the request says so. It ends with a space for the head's approval.
Request for a school transcript or a duplicate certificate (Romania)
The request to a Romanian school for a transcript — the marks for each year of study — or a duplicate of a lost or damaged certificate: the years and class, the document wanted, its purpose and the name used while studying. For a duplicate of a lost diploma, the loss is first published in the Monitorul Oficial and a notarial declaration made, and the request attaches both.
Claim against the at-fault driver's motor insurer (Romania)
The claim in which someone who suffered loss in a road accident in Romania asks for compensation directly from the RCA insurer of the vehicle at fault, without going through its driver (Law no. 132/2017): the date, place and course of the accident, the at-fault policy, the damaged vehicle and the loss, the documents attached and the account for payment.
Claim under private health insurance (Romania)
The claim to an insurer in Romania for medical costs covered by a private health policy: the policy, the event, the healthcare provider and treatment period, the sum claimed and the documents attached — medical and payment ones. A refusal must be reasoned, and the next step is ASF, SAL-FIN or the courts.
Request for access to medical records (Romania)
The request by which a patient in Romania obtains copies of their own medical records from a hospital, clinic or practice — the case notes, the discharge letter, test results, imaging — under Law no. 46/2003 and art. 15 of the General Data Protection Regulation. The first copy is free, and the provider answers without delay and within a month, extendable by up to two more months only where the complexity or number of requests makes it necessary.
Freedom of information request (Romania)
The request by which anyone in Romania obtains information from a public body about its work — spending, contracts, decisions, organisation — without having to say why (Law no. 544/2001). The body answers within 10 days, 30 at most if the information is complex, and a refusal must be reasoned within 5 days. The request says in what form the answer is wanted.
Request for child-raising leave (Romania)
The request to a Romanian employer for child-raising leave until the child turns 2, or 3 for a child with a disability (Emergency Ordinance no. 111/2010), with the employment contract suspended for that time. It also asks for the income certificate needed for the monthly allowance claim, which is filed separately, at the town hall.
Request for an employment and seniority certificate (Romania)
The request by which an employee or former employee in Romania obtains the certificate of the work done, its length, the salary and seniority in work, trade and specialty (art. 34(5) of the Labour Code). Since the paper work book was abolished in 2011, this is the proof of seniority for a new job, a competition or a pension file.
Annual leave request (Romania)
The request in which an employee in Romania asks to take annual leave: the dates, the number of working days, the year it belongs to and the colleague covering the duties. It ends with a space for the employer's approval. Annual leave is at least 20 working days, and holiday pay is paid before the leave begins.
Unpaid leave request (Romania)
The request for unpaid leave to deal with personal matters (art. 153 of the Romanian Labour Code): the dates and the reason. The leave is granted with the employer's agreement, and during it the employment contract is suspended — no salary and no contributions are paid. The maximum length is set by the collective agreement or the internal rules.
