Special power of attorney for a specific act (pełnomocnictwo szczególne)
A Polish power of attorney for an act beyond ordinary management — selling a car, signing a particular contract, collecting money, handling a specific matter — setting out the act, its limits, how long it lasts, and whether the agent may appoint a substitute or contract with themselves. Where the act needs a special form, the power of attorney needs the same one — selling real estate requires a notarial deed, as the document warns.
Revocation of a power of attorney (odwołanie pełnomocnictwa)
A principal's statement revoking a Polish power of attorney, saying when it was given and what it covered, demanding the document back (art. 102 of the Civil Code) and listing the people and institutions told of the revocation. It takes effect when it reaches the agent, but an act done with someone unaware of the revocation can still stand — so the bank, the office and business partners should be told too.
General power of attorney (pełnomocnictwo ogólne)
A Polish power of attorney for acts of ordinary management — running the principal's day-to-day affairs and property: paying bills, collecting post, managing a let flat, dealing with offices. It must be in writing on pain of nullity (art. 99 § 2 of the Civil Code). The document also states what it does not cover — selling property, loans, gifts — since those need a power of attorney naming the type of act or the act itself.
Consent for a child to travel abroad (zgoda na wyjazd dziecka za granicę)
A parent's written consent for a minor to travel abroad from Poland — with the other parent, grandparents, a camp or anyone else: the parent's and child's details, the travel document, countries, dates and the adult in charge. Polish law sets no official form, but parents decide important matters for a child together, and border guards may check consent when a child travels without one parent. There is room below the signature for a notary's certification.
Parental consent to a child's medical care while with another carer
A parent's written consent in Poland to examinations and treatment of a child while someone else looks after them — grandparents, a camp leader, a school trip — authorising that person to receive health information and collect records. A carer who is not a parent can only agree to an examination on their own, and higher-risk procedures need the parent's separate consent; the document says so plainly and gives the parent's phone number and the child's allergies and medicines.
Request to work remotely (wniosek o pracę zdalną)
An employee's request in Poland to work remotely in full or in part: from when, where and on what pattern, with a statement about premises and equipment. Some employees — pregnant ones and parents of children up to 4, among others — are entitled to have the request granted unless the organisation or type of work rules it out; the letter lets you state that ground. Occasional remote work, up to 24 days a year, has its own request.
Post-employment non-compete agreement (zakaz konkurencji)
An agreement under which an employee with access to particularly important information undertakes not to compete with the employer for a set time after leaving. The employer pays compensation of at least 25% of the pay received before the employment ended, for a period equal to the ban. It must be in writing on pain of nullity, and stops binding when the employer stops paying.
Demand to an employer for unpaid wages
An employee's demand for unpaid wages in Poland: for which periods, how much, by when and to which account, reminding the employer that pay is due within the first 10 days of the following month, and saying that the labour court and the National Labour Inspectorate come next. It ends the silence and proves an attempt to settle amicably.
Parental leave request (wniosek o urlop rodzicielski)
An employee's request for parental leave in Poland: the child and date of birth, the length in weeks, the dates, what the other parent has taken, and the attachments. Parental leave is 41 weeks for both parents together (43 for more than one child), 9 weeks of which are reserved for each parent. The request goes in at least 21 days before the leave starts.
Request to an employer for a reference letter
A courteous, practical request to a current or former employer in Poland for a reference: the position and period worked, what it is needed for and what is worth highlighting. In Poland a reference is voluntary — only the certificate of employment is compulsory — so the letter makes agreeing easy by suggesting specific achievements and a date.
Request to correct a certificate of employment (świadectwo pracy)
A request to the employer to correct errors in the Polish certificate of employment — the wrong period of employment, the wrong way the contract ended, the wrong amount of leave taken. It is made within 14 days of receiving the certificate; if the employer refuses, a claim can go to the labour court within 14 days of being told. The letter says what is wrong, what it should say, and why.
Mutual agreement ending an employment contract (porozumienie stron)
Ending a Polish employment contract by agreement of both sides, on a date they choose, with no notice period and no reasons given. The agreement records who proposed it — which matters for the certificate of employment, for severance where the reasons do not concern the employee, and for the labour office — and settles leave and anything else the parties have agreed.
Unpaid leave request (wniosek o urlop bezpłatny)
An employee's written request for unpaid leave in Poland: dates, number of days and reason. The employer may grant it but does not have to; there is space for the decision on the same sheet. Unpaid leave does not count towards the service on which employee entitlements depend, and for leave over three months the parties can agree that the employee may be recalled earlier for important reasons.
Annual leave request (wniosek o urlop wypoczynkowy)
An employee's request for annual leave in Poland: for which year, how many working days and when, with space for the employer's decision on the same sheet. Leave is 20 days with under 10 years' service and 26 days with at least 10 years, and time in education counts towards that. Leave on demand — up to 4 days a year — is requested on the day it starts at the latest.
Permanent employment contract (umowa o pracę na czas nieokreślony)
A Polish employment contract with no end date, carrying what art. 29 § 1 of the Labour Code requires: the parties with the employer's registered address, job title and duties, place of work, working time, pay and its components, pay date and start date. Notice periods are written out, and the employer gives the remaining terms separately within 7 days of the employee starting work.
Fixed-term employment contract (umowa o pracę na czas określony)
A Polish employment contract with an end date, carrying the elements art. 29 § 1 of the Labour Code requires and the limit in art. 25¹: no more than 33 months and three such contracts in total between the same parties — beyond that the contract becomes a permanent one. Since 2023 the employer's notice must state its reason, and the notice periods are the same as for a permanent contract.
Employee's resignation letter (wypowiedzenie umowy o pracę)
Notice of resignation from a Polish employment contract, with no reason needed: which contract, what notice period and when it ends — a period counted in weeks ends on a Saturday, one counted in months on the last day of a month. The letter also asks for the certificate of employment on the last day and for leave to be settled. If both sides want to part sooner, they can agree an earlier date after notice is given.
Request for the landlord's consent to sublet a flat
A tenant's request in Poland for written consent to sublet the flat or part of it: to whom, for how long, for what rent and why, with confirmation that the tenant's obligations to the landlord stay with the tenant. There is room at the foot for the landlord's signature — a signed copy is the proof of consent, because subletting without it is one of the grounds on which the landlord may end the tenancy with a month's notice.
Demand for return of a rental deposit
A tenant's letter to a landlord in Poland who has not returned the deposit after moving out: when the flat was vacated, how much is to be repaid, by when and to which account, with a request for a written account of every deduction. The landlord has a month from the flat being vacated; after that interest runs, and the letter also shows the attempt at an amicable solution a court will ask about.
Demand to the landlord to repair defects in a flat
A tenant's letter in Poland reporting defects the landlord is responsible for — failed heating, a leak, a broken boiler — setting a deadline for the repair and asking for a rent reduction while the defects last. The deadline is what later allows the tenant to have the repair done at the landlord's expense (art. 663 of the Civil Code); the notice also opens the way to ending the tenancy if the defects cannot be fixed.
Notice terminating the current rent (rent increase)
A rent increase for a flat in Poland done the way the law requires: not as a "notice of increase" but as written termination of the current amount of rent, with three months' notice (or longer under the agreement), to the end of a calendar month. The letter states the new rent and the date it applies from, and tells the tenant their rights — which is fair to them and makes the increase easier to defend.
Flat handover record — move-out
The record made when a rented flat in Poland is returned at the end of the tenancy: the state of the flat and furnishings compared with the move-in record, meter readings, keys returned, damage found and the deposit settlement — how much is deducted, for what, and to which account the rest goes back. Settling it on the spot is the simplest way to keep the deposit out of court.
Flat handover record — move-in
The record made on the day a flat in Poland is handed to the tenant: the state of the flat, its furnishings, the electricity, gas, water and heating meter readings, the keys handed over and any defects noticed, signed by both sides. When the flat is returned its state is compared with this record — which is what decides which damage happened during the tenancy, and so what may be deducted from the deposit.
Landlord's notice ending a flat tenancy
Notice ending a Polish residential tenancy with one month's notice to the end of a calendar month, on the grounds in art. 11(2) of the tenants' protection act: arrears after a written warning, use against the agreement despite a warning, subletting without consent. The letter states the reason, because without it the notice is void. An owner wanting to move in relies on a different ground with longer periods, and this is not that document.
