Application for a duplicate document
Used where an original document has been lost, stolen or destroyed — a diploma, a certificate, an employer's letter. The particulars of the lost document and the reason for the loss are stated.
Freedom of information request
Every institution is obliged to answer a request like this within fourteen days. The form follows Art. 24 of the Access to Public Information Act and carries every particular the Act requires, so that it cannot be set aside without consideration.
Receipt for money received
Written evidence that a sum has been paid and received — on a private sale, a deposit, rent or a loan. It is completed by the person receiving the money and signed as the money changes hands.
Declaration under Art. 313 of the Criminal Code
The all-purpose declaration that institutions and employers ask for most often. You fill in your details and the wording of what you are declaring; the clause on criminal liability for a false declaration is already in the text.
