Contract for work (civil contract)
The contract between a client and a contractor for a specific piece of work — a repair, a translation, a design, a course, anything given to a person rather than to a company. The weak point of most such contracts is vagueness: without a precise description of the work, a deadline and criteria for acceptance, there is no way to prove what was agreed. Here all three are separate fields rather than free text.
Parent's application for a pupil's absence
The application by which a parent excuses a child's absence for family reasons. How many days a parent may excuse themselves, and from which point the head teacher's permission is needed, is set by each school's own rules — check yours before filling in the dates. It is filed in advance, not after the child is back in class.
Sale of movable property
For a private sale of equipment, furniture, tools, instruments — anything handed over by hand. The contract describes the item, the price in words and in figures, and the seller's declaration that the item is theirs and is not pledged. Note that it will not do for a motor vehicle: there the law requires the signatures to be notarised.
Objection to a utility bill
A bill for electricity, water or heating that does not match actual consumption is challenged in writing with the supplier, before paying under protest or letting it reach disconnection. The objection identifies the invoice, the meter reading and the ground. Keep proof that you filed it — that is what halts further action on the account.
Request for a reference from an employer
A reference is not a certificate: it is an assessment of your work, and an employer is not obliged to give one. So the request states which period, for what purpose and in which language you need it — a specific request is answered far more often than a general one.
Application to change the bank account for salary payments
Changing banks is a small matter until a salary goes to a closed account. This application tells the payroll office what the new account is and from which month it applies. File it before payday rather than after — the transfer is prepared in advance.
Notification to the employer of absence through illness
A sick note is issued by the doctor, but the employer has to be told that you are absent — at once, not when you come back. This document does that in writing: the date your absence starts, the number of the sick note, and how to reach you. The notification does not replace the sick note; it goes with it.
Notice of withdrawal from a distance contract
The right to return an online purchase within fourteen days without giving a reason. The period runs from when the goods were received, not from the order, and it is met if you send the notice within it — even if the goods start their journey back later. The form identifies the order and the account the refund goes to.
Complaint about faulty goods
When something you have bought breaks, or turns out not to be what was promised, the trader owes you a repair, a replacement or your money back — but by law the complaint has to be made in writing. This form does that: what you bought, when, what is wrong with it and what you are asking for. Keep a copy with a reference number, or proof of posting; without evidence that you made the complaint, time does not run in your favour.
Owner's consent for address registration
When you register at an address that is not yours, the municipality asks for the owner's written consent. This is that document. It goes in together with the application for a permanent or current address, and without it the counter returns the whole file. If the owner will not be coming to the counter in person with their ID card, the signature has to be notarised — which is why most people end up going twice.
Consent to act as managing director and specimen signature
The document by which a managing director accepts the post and gives the specimen signature the Commercial Register will hold. Its whole point is the notarisation — the signature is given in front of a notary, not at home. The lower part of the sheet is left blank for exactly that; do not fill it in.
Declaration under Art. 141(8) of the Commerce Act
The declaration every new managing director files — that they meet the requirements of the law and that nothing bars them from holding the post. It is filed when the company is incorporated and on every later change of managing director.
Declaration under Art. 13(4) of the Commercial Register Act
A mandatory attachment to every filing with the Commercial Register: that the circumstances declared exist and that the documents attached are genuine. It is short, but a filing does not go through without it — and nowhere is it explained who exactly signs it. The applicant does, meaning the person making the filing.
Resolution of the sole owner of the capital
The resolution by which the owner incorporates the company, adopts the constitutive act, fixes the registered seat and the capital, and appoints a managing director. It is filed together with the constitutive act and repeats the same particulars — fill both in at one sitting so that they cannot disagree.
Constitutive act of a single-member limited company (EOOD)
The founding document of the company: name, registered seat, objects, capital, shares, managing director and the manner of representation. It is filed with the Commercial Register together with the rest of the pack. The minimum capital is one euro. Before filling in anything, check that the company name is free in the Commercial Register — if it is taken, the whole pack has to be redone.
Request for a certificate of income
The bank, the court or the social services office wants a document showing your income, and the employer wants a written request. This form is that request: for which period, for what purpose, in how many copies. Stating the purpose matters — a certificate for a loan and a certificate for social assistance often carry different figures.
Consent for a child to travel abroad
When a child leaves the country with only one parent or with another adult, border control asks for the other parent's written consent. The declaration names the countries, the period and the accompanying adult by name. The signature has to be notarised — without that the document is of no use at the border. We prepare the wording; the notarisation is done at a notary.
Loan agreement between private individuals
When you lend money to a relative or a friend, this is the document that turns a spoken arrangement into an obligation that can be proved. It records the amount in figures and in words, when it was handed over and how, when it is repaid, whether there is interest and what is owed on late payment. The amount in words is not a formality: where the two disagree, a court reads the words.
Notice to terminate a tenancy
The written notice by which a landlord or a tenant brings a tenancy to an end. It works from either side — you state which of the two you are. The notice period is the one in your own agreement; the termination date runs from service, not from signature. Deliver it in a way that lets you prove when it was received.
Property handover record
The document that decides whose the deposit is. It records the readings of every meter, the number of keys handed over, the contents and the condition of the property at that moment. It is completed both on moving in and on moving out — comparing the two records is the only objective evidence of what changed.
Power of attorney (general)
The document by which you authorise someone else to do something in your place — to file an application, to collect a document, to represent you before an institution. You list the powers one by one and set a time limit. Note that for dealing with real property, with a vehicle, and in a number of proceedings before state bodies, the signature has to be notarised. We prepare the document; the notarisation is done at a notary.
Application to terminate employment by mutual consent (Art. 325(1)(1) of the Labour Code)
The way to leave at once, without working a notice period — but only if the employer agrees. The application proposes a date on which the contract is to end. The part almost nobody knows: the employer must answer within seven days, and silence counts as a refusal. Without written consent there is no termination.
Notice of resignation (Art. 326 of the Labour Code)
When you leave of your own accord, the law wants written notice — not a conversation and not an email. This document puts it in the form Art. 326(1) of the Labour Code requires, states the notice period and names your last working day, so there is no argument about when the contract ends. The period is thirty days unless your contract says otherwise, and by law it cannot exceed three months.
